Abstract

A osteoartrite é a doença articular degenerativa crónica mais comum, sendo a principal causa de dor e incapacidade física, afetando milhões de pessoas em todo o mundo. É, principalmente, caracterizada por ser uma patologia que envolve toda a articulação, incluindo a degradação da cartilagem, a remodelação óssea, a formação de osteófitos e a inflamação sinovial, o que resulta em dor, rigidez, inchaço e perda da função normal da articulação. É uma doença multifatorial, com etiologia que varia desde o processo normal de envelhecimento, sexo, obesidade e até fatores físicos, como traumas e lesões nas articulações, existindo uma forte interação entre todos esses fatores. Apesar da alta prevalência da osteoartrite, ainda não existe um medicamento modificador da doença aprovado pelas agências reguladoras, que reduza os sintomas e atrase a progressão da doença. O tratamento atual inclui intervenções não farmacológicas, farmacológicas e cirúrgicas. Dado o facto de que os tratamentos farmacológicos existentes carecem de propriedades modificadoras da doença e o seu uso tem associado efeitos secundários significativos, os nutracêuticos com compostos bioativos constituem um campo relevante de pesquisa. Os polifenóis são metabolitos secundários que estão amplamente presentes nas raízes, caules, folhas, flores, frutos e sementes das plantas. Estes têm sido extensamente estudados devido às suas propriedades antioxidantes, anti-inflamatórias e imunomoduladoras em muitas condições crónicas inflamatórias. Tem sido demonstrado que os polifenóis têm diversos efeitos benéficos em várias doenças, como cancro, obesidade, diabetes, doenças cardiovasculares, distúrbios neurodegenerativos e, também, em doenças reumáticas. Nesta monografia, pretende-se apresentar uma visão geral da osteoartrite e obter um maior conhecimento sobre alguns dos polifenóis que apresentam potencial terapêutico no tratamento da osteoartrite, nomeadamente, a curcumina, quercetina, resveratrol e epigalocatequina galato.

Osteoarthritis is the most common chronic degenerative joint disease, being the leading cause of pain and physical disability, affecting millions of people worldwide. It is primarily characterized as a condition that involves the entire joint, including cartilage degradation, bone remodeling, osteophyte formation, and synovial inflammation, resulting in pain, stiffness, swelling, and loss of normal joint function. It is a multifactorial disease with etiology ranging from the normal aging process, gender, obesity, to physical factors such as trauma and joint injuries, with a strong interplay among all these factors. Despite the high prevalence of osteoarthritis, there is still no disease-modifying drug approved by regulatory agencies that reduces symptoms and delays disease progression. Current treatment includes non-pharmacological, pharmacological, and surgical interventions. Given the fact that existing pharmacological treatments lack disease-modifying properties and are associated with significant side effects, nutraceuticals with bioactive compounds constitute a relevant field of research. Polyphenols are secondary metabolites widely present in the roots, stems, leaves, flowers, fruits, and seeds of plants. They have been extensively studied due to their antioxidant, anti-inflammatory, and immunomodulatory properties in various chronic inflammatory conditions. Polyphenols have demonstrated diverse beneficial effects in various diseases, including cancer, obesity, diabetes, cardiovascular diseases, neurodegenerative disorders, and rheumatic diseases. This monograph aims to provide an overview of osteoarthritis and gain a deeper understanding of some of the polyphenols that have therapeutic potential in osteoarthritis treatment, including curcumin, quercetin, resveratrol, and epigallocatechin gallate.
