Abstract

A multimorbilidade (duas ou mais condições coexistentes num indivíduo) é um desafio global crescente com efeitos substanciais sobre os indivíduos, cuidadores e a sociedade. A maioria das normas de orientação clínica, formação e prestação de cuidados de saúde concentram-se em doenças individuais, levando a cuidados por vezes inadequados e potencialmente prejudiciais. A multimorbilidade requer cuidados centrados na pessoa, priorizando o que é mais importante para o indivíduo e para os seus cuidadores, garantindo que o atendimento é coordenado de forma eficaz e com o mínimo de interrupções, alinhando-se com os valores do paciente. A necessidade de orientar doentes com multimorbilidade é um dos desafios da medicina geral e familiar. As limitações funcionais consequentes da diabetes mellitus tipo 2 e de outras doenças crónicas exigem uma boa articulação de cuidados. O médico de família, que melhor enquadra o contexto biopsicossocial do doente, deve assumir um papel primordial nesta integração. Relata-se o caso de um homem de 45 anos, com antecedentes de talassémia minor, asma persistente leve, esteatose hepática não alcoólica, diabetes mellitus tipo 2 com complicações microvasculares nomeadamente neuropatia, nefropatia e retinopatia, hipertensão arterial e dislipidémia. Apresentam-se os contributos para um plano individual e integrado de cuidados. Com o presente caso, salienta-se a importância da implementação de um modelo de cuidados centrado na pessoa, integrando toda a constelação da sua multimorbilidade.

Multimorbidity (the presence of two or more coexisting conditions in an individual) is a growing global challenge with substantial effects on individuals, caregivers, and society. Most clinical guidelines, training, and healthcare delivery focus on individual diseases, leading to sometimes inadequate and potentially harmful care. Multimorbidity requires person-centered care, prioritizing what matters most to the individual and their caregivers, ensuring that care is effectively coordinated with minimal disruptions, and aligning with the patient's values. The need to guide patients with multimorbidity is one of the challenges in general and family medicine. The functional limitations resulting from type 2 diabetes mellitus and other chronic diseases demand well-coordinated care. The family physician, who better understands the patient's biopsychosocial context, must play a primary role in this integration. We report the case of a 45-year-old man with a history of minor thalassemia, mild persistent asthma, non-alcoholic fatty liver disease, type 2 diabetes mellitus with microvascular complications, including neuropathy, nephropathy, and retinopathy, hypertension, and dyslipidemia. The contributions to an individualized and integrated care plan are presented. With this case, the importance of implementing a person-centered care model is emphasized, integrating the entire constellation of his multimorbidity.
