Abstract

Introdução/Objetivos: O envelhecimento afeta os músculos da mão de forma não uniforme. Avaliou-se a influência do envelhecimento na amplitude da resposta motora (CMAP) dos músculos da mão e no, correspondente, split-hand index (SHI). Métodos: Foram revistos os dados de 244 indivíduos com a suspeita de miastenia ocular e nos quais foram excluídas doenças neuromusculares. As respostas motoras dos músculos curto abdutor do polegar (APB), primeiro interósseo dorsal (FDI) e abdutor do quinto dedo (ADM) foram obtidas por estimulação elétrica no punho e registadas por elétrodos superficiais, como parte do protocolo do estudo neurofisiológico. Foram aplicados os testes de Tukey HSD e de Kruskal-Wallis para comparar os vários grupos etários, definidos pela mediana e pelo intervalo interquartil (IQR). O coeficiente de correlação de Spearman e a regressão linear foram utilizados para avaliar a relação entre a idade e os valores medidos. Resultados: A mediana da idade foi de 61.5 anos (1º IQR 44.5, 3º IQR 72.0, 18-89 anos). Os valores de idade e os valores da amplitude foram semelhantes em ambos os sexos. Verificou-se uma correlação entre a idade e os valores de APBCMAP, FDICMAP, ADMCMAP e do SHI (p <0.001). A mediana e os valores cut-off foram significativamente diferentes entre os grupos etários. Os valores de APBCMAP, FDICMAP e ADMCMAP apresentaram uma redução de 0.08/0.07/0.03 mV/ano, respetivamente. Verificou-se que os valores do SHI tiveram uma redução de 0.15/ano. Conclusões: A resposta motora dos músculos da mão e o SHI correspondente são fortemente influenciados pelo envelhecimento, apesar deste efeito ser menor no ADM. Tal facto representa um processo fisiológico. Estudos futuros com recurso ao SHI deverão considerar o efeito do envelhecimento.

Introduction/Aims: Age can affect hand muscles non-uniformly. We investigated the influence of age on the compound muscle action potential (CMAP) amplitude of the hand muscles and the derived split-hand index (SHI). Methods: We studied 244 subjects investigated for suspected myasthenia gravis but without neuromuscular disorders. Abductor pollicis brevis (APB), first dorsal interosseous (FDI), and abductor digiti minimi (ADM) CMAPs were obtained by supramaximal stimulation at the wrist, recording with surface electrodes while checking the best recording site. We applied Tukey's HSD and Kruskal–Wallis one-way analysis of variance for comparing age groups defined by median and interquartile ranges (IQRs). Spearman's rank correlation coefficient and linear regression were used for testing the age-dependence of measurements. Results: Median age was 61.5 y (first IQR, 44.5; third IQR, 72.0; range 18–89). Age and neurophysiological measurements were similar between genders. APBCMAP, FDICMAP, ADMCMAP, and SHI were correlated with age (P < .001). Median and cutoff values were significantly different between age groups. APBCMAP, FDICMAP, and ADMCMAP decreased by 0.08/0.07/0.03 mV/y, respectively, and SHI decreased 0.15/y. Discussion: The CMAP amplitudes of hand muscles and derived SHI were strongly age-dependent, although this effect was less in ADM. This represents a physiological phenomenon. Future studies using the SHI should consider age effects.
