Cuidados de conforto : as perceções dos profissionais de saúde

Abstract

Diariamente nos serviços de internamento hospitalar, os profissionais de saúde são confrontados com doentes com patologias de evolução prolongada que agudizam, transformando-se algumas em complicações irreversíveis. A identificação destes doentes por vezes não é clara nem linear, pelo peso ainda sentido pelos profissionais no abandono do modelo curativo. A um doente considerado para “apenas medidas de conforto”, que medidas é que são iniciadas e/ou mantidas? Para responder a esta questão foi realizado um estudo qualitativo, com a realização de entrevistas semi-estruturadas a profissionais de saúde que exercem funções em unidades de cuidados intensivos e enfermarias de medicina interna, 15 enfermeiros e 11 médicos. Estes profissionais apontaram 22 atitudes terapêuticas a manter/iniciar/suspender/não iniciar, dessas apenas sete foram referidas por mais de metade da amostra: i) controlo da dor 85%; ii) comunicação entre a equipa multidisciplinar 73%; iii) suspender avaliação de parâmetros analíticos 69%; iv) não escalar antibioterapia, em caso de re-infeção 65%; v) manter/iniciar o que contribui para o conforto 62%; vi) controlo da dispneia com recurso a oxigénio por óculos nasais e/ou máscara facial 54%; vii) não instituir alimentação/hidratação artificial, após a perda de via oral 50%. A classe profissional e a tipologia de serviço têm influência na tomada de decisão, mas não de uma forma significativa, a variação com maior expressão foi a formação em cuidados paliativos.

Healthcare professional face daily challenges while trying to look after severely unwell patients, that deteriorate dramatically, so much that their status becomes irreversible. However, healthcare professionals are not yet prepared to abandon the biomedical model, because the process of identifying these patients is not yet clear to them, and they still see as burden. Patients who are considered just for “comfort measures only”, what measures should be started or kept? To answer this research question, a qualitative study was done using semi-structured interviews of 15 nurses and 11 doctors working in Intensive Care Units and Internal Medicine wards. Healthcare professionals pointed out 22 end of life care measures that should not be started/ should be kept/ should be started and should be stopped. However, only 7 out of 22 were mentioned by more than half of the sample: i) pain management, 85%; ii) multidisciplinary communication, 73%; iii) stop monitoring vital signs, 69%; iv) not to escalated antibiotics, if new infection, 65%; v) keep/ start comfort measures, 62%; vi) the use of oxygen, through nasal canula or facemask, to control dyspnoea, 54%; vii) not to start enteral feeding or parenetic nutrition or hydration, in patients unable to swallow, 50%. Decision making is affected by the different classes and their place of work, but not in a significant way. Formation in palliative care is the most signify variable.
