Abstract

Introdução: Os cuidados paliativos são cuidados de saúde especializados para pessoas com doenças graves e/ou avançadas e progressivas, sendo transversais a todas as áreas de cuidados. São cada vez mais reconhecidos como componente integral na abordagem de doentes críticos, intervindo em aspetos como: a prevenção e alívio do sofrimento físico, psicológico, social e espiritual dos doentes e famílias; a adequação dos objetivos dos cuidados de acordo com a condição clínica, o prognóstico e as necessidades dos doentes; a comunicação empática e gestão das expectativas. Objetivos: Pretende-se com este trabalho salientar a evidência atual no que concerne à identificação precoce dos doentes com necessidades paliativas, às particularidades da abordagem destes doentes, às principais estratégias na tomada de decisão e comunicação entre profissionais de saúde, doentes e familiares, e à perspetiva dos intensivistas acerca da integração desta área nas unidades de cuidados intensivos. Material e métodos: Revisão narrativa da literatura mais recente referente à aplicação dos cuidados paliativos nas unidades de cuidados intensivos, priorizando as guidelines internacionais e artigos de revisão. Resultados: Foram identificados 18 artigos considerados pertinentes para esta revisão, incluindo oito revisões sistemáticas, oito revisões narrativas, um questionário e um consenso. Discussão: Esta revisão narrativa realça a importância dos Cuidados Paliativos na abordagem de sintomas, na comunicação e na tomada de decisão partilhada assim como as estratégias e o impacto da sua implementação nas Unidades de Cuidados Intensivos. Conclusão: A integração dos cuidados paliativos nas Unidades de Cuidados Intensivos é um processo que poderá melhorar a qualidade dos cuidados, diminuir a mortalidade e os custos económicos, e aumentar os níveis de satisfação dos profissionais de saúde, doentes e famílias.

Introduction: Palliative care is specialized medical care for people living with serious and/or advanced and progressive diseases, being transversal to all areas of care. They are increasingly recognized as an integral component in the approach of critically ill patients, intervening in aspects such as: the prevention and relief of the physical, psychological, social and spiritual suffering of patients and families; the adequacy of care objectives according to the clinical condition, prognosis and needs of patients; empathic communication and management of expectations. Objectives: The aim of this study is to highlight the current evidence regarding the early identification of patients with palliative needs, the particularities of the approach of these patients, the main strategies in decision-making and communication between health professionals, patients and family members, and the perspective of intensivists about the integration of this area in intensive care units. Methods: Narrative review of the most recent literature on the application of palliative care in intensive care units, prioritizing international guidelines and review articles. Results: Eighteen articles were identified and considered relevant for this review, including eight systematic reviews, eight narrative reviews, a questionnaire and a consensus report. Discussion: This narrative review highlights the importance of Palliative Care intervention in symptom approach, communication and shared decision-making as well as the strategies and impact of their implementation in Intensive Care Units. Conclusion: The integration of palliative care into Intensive Care Units is a process that can improve the quality of care, reduce mortality and economic costs, and increase the levels of satisfaction of health professionals, patients and their families.
