Utilidade da ecografia com contraste no estudo da sinovite : uma revisão sistemática

Abstract

Introdução Apesar da crescente evidência acerca da ecografia com contraste (CE-US) e da sua aplicação clínica nas doenças músculo-esqueléticas, a sua utilidade ainda carece de evidência robusta, bem como de uma padronização clinicamente válida para a sua aplicação, particularmente nas doenças inflamatórias articulares. Objectivos Sistematizar a melhor evidência disponível acerca da utilização da CE-US nas doenças inflamatórias articulares, de modo a compreender a validade da sua aplicabilidade e identificar as situações que carecem de evidência sólida. Métodos de pesquisa Fez-se pesquisa na base de dados Pubmed, incluindo as publicações até Novembro de 2021, inclusive. Critérios de selecção Estudos clínicos escritos em língua inglesa acerca da utilização da CE-US na detecção de sinovite para o diagnóstico, monitorização e avaliação da resposta à terapêutica em doenças reumáticas inflamatórias. Resultados Apesar de alguma discrepância entre os resultados publicados, parece haver um aumento da sensibilidade na detecção de sinovite com a administração de contraste na ecografia articular, possivelmente à custa de diminuição da especificidade. Conclusões A ecografia com contraste é considerada uma técnica promissora no diagnóstico e seguimento dos doentes com patologia reumática. Estão em falta estudos consistentes que comparem esta às outras modalidades de imagem disponíveis Ainda não existe evidência suficiente para que se estabeleçam recomendações claras para a utilização de CE-US no estudo da sinovite.

Background While rheumatology experts have been witnessing a growing body of evidence on constrast-enhanced ultrasound (CE-US) and its clinical use in musculoskeletal diseases, it still lacks the appropriate evidence supporting its use in the various clinical settings as well as a standardization with proven validity for its application, particularly in inflammatory joint diseases. Objectives To summarize the best available evidence regarding the use of CE-US in inflammatory joint diseases in order to assess its validity and identify which situations lack solid evidence. Search methods Pubmed database was searched up to and including November 2021. Selection criteria Clinical studies in English of the use of CE-US in the detection of synovitis for the diagnosis, follow-up, and evaluation of therapeutic response of inflammatory joint diseases. Results Despite contradictory evidence, there’s a tendency for increase in sensitivity with the administration of contrast in ultrasound examination of the joints, possibly at the expense of a reduced specificity in the study of synovitis. Conclusions Contrast enhanced ultrasound is a promising diagnostic and follow-up technique in rheumatic diseases. Further consistent investigation comparing this to other available imaging modalities is needed. There is not enough evidence to establish recommendations concerning the use of CE-US in the study of synovitis.
