Abstract

A IMA é uma emergência médica e cirúrgica que requer um tratamento emergente, dependente da etiologia da disfunção valvular. Uma das causas de IMA, com maior mortalidade, é a isquemia, devido a EAM, com RMP, muitas vezes associado. Métodos: Este trabalho pretende verificar os resultados do tratamento contemporâneo da IMA de causa isquémica. No enquadramento deste tema, considerou-se relevante a abordagem de uma série de 10 casos, 9 doentes do sexo masculino e 1 doente do sexo feminino, sendo a média de idade de 64,3 ± 8,5 anos. Nesta série de casos serão analisados os antecedentes pessoais e médico-cirúrgicos, a apresentação clínica, o diagnóstico, o tratamento, dando ênfase ao tratamento cirúrgico (realização valvuloplastia ou substituição valvular, associado a revascularização ou não), à evolução clínica no internamento, à morbilidade, à mortalidade e ao prognóstico dos doentes, seguindo-se uma breve discussão sobre o assunto. Resultados: Observou-se que 80% dos doentes tiveram RMP. Verificou-se que todos os doentes realizaram no mínimo um procedimento de revascularização do miocárdio, PCI ou CABG, e 70% dos doentes realizaram CABG concomitante com cirurgia da VM. 30% dos doentes realizaram anuloplastia da VM e 70% colocaram uma prótese na VM. Sobreviveram 20% dos doentes, todos realizaram anuloplastia. Conclusão: Os resultados do tratamento cirúrgico, nesta série, foram pouco favoráveis, pois verificou-se uma mortalidade elevada. Não existe um tratamento consensual para esta patologia, contudo, o CABG concomitante com a cirurgia da VM tem um melhor prognóstico a curto e a longo prazo.

Introduction: AMI is a medical and surgical emergency that requires emergent treatment, which depends on the valvar dysfunction etiology. Ischemia, due to acute myocardial infarction, is one of the causes of AMI, with higher mortality. It is often associated with PMR. Methods: This study aims to evaluate the results of contemporary treatment of ischemic AMI. In the context of this theme, we studied a series of 10 cases, 9 male patients and 1 female patient, with a mean age of 64,3 ± 8,5 years. In this case series we will analyze personal, medical, and surgical history, clinical presentation, diagnosis, treatment, focusing on surgical treatment (valvuloplasty or valve replacement, associated with revascularization or not), clinical evolution during hospitalization, morbidity, mortality, and prognosis of patients, followed by a brief discussion on the subject. Results: It was observed that 80% of patients had PMR. All patients went through at least one revascularization procedure, PCI or CABG, and 70% of patients underwent CABG concomitantly with MV surgery. 30% of patients had MV annuloplasty. 20% of the patients survived, all of them underwent annuloplasty. Conclusion: In this series, the results of surgical treatment were adverse, because there was a high mortality rate. There is no consensual treatment for this pathology, however, in moderate to severe cases, CABG concomitant with MV surgery has a better short- and long-term prognosis.
