Abstract

A Ectrodactilia é uma malformação congénita rara que é caracterizada pela ausência de um ou mais raios centrais da mão ou do pé, adquirindo o aspecto típico de “garra de lagosta”. É uma doença com transmissão autossómica dominante que pode estar associada a outras malformações congénitas. A Ectrodactilia continua a ser um desafio clínico, contando com diversas dificuldades no aconselhamento genético devido à penetrância incompleta e à expressão variável da doença e na escolha da abordagem cirúrgica apropriada pela grande variabilidade fenotípica e escassez de evidência nas técnicas cirúrgicas publicadas na literatura. Neste contexto, é apresentado e discutido um caso clínico de um doente em idade pediátrica com ectrodactilia do pé esquerdo que foi submetido ao tratamento conservador com ortóteses e, posteriormente, à reconstrução cirúrgica da deformidade do pé. Abordadas as problemáticas inerentes aos testes genéticos e tratamento, contextualizouse o caso clínico com base na revisão bibliográfica efetuada.

Ectrodactyly is a rare congenital anomaly that is characterized by the absence of one or more of the central rays of the hand or foot, acquiring the typical “lobster-claw” aspect. It is a disease with autosomal dominant transmission that can be associated with other congenital malformations. Ectrodactyly remains a clinical challenge, with several difficulties in genetic counseling due to incomplete penetrance and variable expression of the disease and the choice of the appropriate surgical approach due to the huge phenotypic variability and lack of evidence in the surgical techniques published in the literature. In this context, a case report is presented and discussed, in which a pediatric patient with left foot ectrodactyly underwent conservative treatment with orthoses and, subsequentely, the surgical reconstruction of the foot deformity. After discussing the problems inherent to genetic testing and treatment, the case report was contextualized based on the literature review.
