Abstract

Post-traumatic vertigo is not yet considered a clinical entity, however it defines a symptom which is common to an heterogeneous group of neuronal and labirynthine injuries due to cranial, cervical and barotraumatisms. There have been many physiopathologies proposed to explain this alteration in the vestibular pathway, which can be central or peripheral ones. In addition to this, we can divide the peripheral physiopathologies in immediate mechanisms (BPPV, Labirynthine concussion, Perilymphatic fistula, Temporal bone fracture, Otolithic disfunction) and late-onset mechanisms (Ménière disease, Cervixogenic vertigo). The aforementioned medical condition is not fatal. However it raises significant implications as far as morbility is concerned, diminishing the patiets’ quality of life whenever left untreated or is given inadequate medical care. This clinical situation is of great importance from a medicolegal point of view due to the damage compesations and the incapacity declarations in the work field. Thus, it is imperative to not only possess knowledge about the physiopathologic mechanisms of the various types of trauma, but also understand the exact circumstances of the traumatic event to have the ability to establish a causal link, which is extremely important in the judicial panorama.

A vertigem pós-traumática não constitui uma entidade clínica, no entanto, define uma manifestação comum a um conjunto heterogéneo de lesões labirínticas e cerebrais decorrentes de traumatismos cranianos, cervicais ou barotraumatismos. Várias têm sido as fisiopatologias propostas para explicar esta alteração da via vestibular, tanto a nível central como a nível periférico, distinguindo-se ainda mecanismos imediatos (Vertigem Posicional Paroxística Benigna, Concussão labiríntica, Fístula perilinfática, Fratura do rochedo, Disfunção otolítica) e mecanismos tardios (Doença de Ménière, Vertigem cervicogénica). Esta condição médica não é fatal no entanto acarreta significativa morbilidade, diminuindo em muito a qualidade de vida dos doentes quando não tratada ou quando o seu tratamento não é adequado. Esta situação clínica adquire especial importância do ponto de vista médico-legal devido às compensações de danos e às declarações de incapacidade no contexto laboral. Assim, é fulcral não só conhecer os mecanismos fisiopatológicos de vertigem pós-traumática nos diferentes tipos de trauma, mas também compreender as circunstâncias exatas do trauma para que seja possível o estabelecimento de um nexo de causalidade, importantíssimo a nível judicial.
