Estenose subglótica pediátrica

Abstract

A Estenose Subglótica é uma entidade relativamente pouco frequente, que afeta com maior incidência a população pediátrica, particularmente os recém-nascidos pré-termo. Na maior parte dos casos, é secundária ao trauma da mucosa laríngea resultante da intubação endotraqueal, a que são submetidos. Contudo, raramente esta patologia também pode ser congénita. A apresentação clínica desta patologia é variável consoante o grau de gravidade e compromisso da via aérea, pelo que pode ser assintomática ou manifestar-se sobretudo por estridor bifásico e outros sinais de dificuldade respiratória. O seu diagnóstico é estabelecido principalmente por métodos endoscópicos, podendo ser complementado por exames de imagem. O tipo de abordagem terapêutica da Estenose Subglótica Pediátrica depende, de igual forma, do grau de gravidade da mesma, podendo ser abordada por técnicas endoscópicas, como a Laringoplastia com Balão, ou por técnicas de cirurgia aberta, como a Cirurgia de Expansão e Reconstrução Laringotraqueal ou Cirurgia de Ressecção Cricotiroideia Parcial.

Subglottic stenosis is a relatively infrequent entity, which mostly affects the pediatric population, particularly preterm newborns. In most cases, it is mainly secondary to trauma to the laryngeal mucosa that results from the endotracheal intubation, to which these newborns are submitted. However, this pathology can rarely also be congenital. The clinical presentation of this pathology varies according to the degree of severity and impairment of the airway, therefore it may be asymptomatic or manifest mostly through biphasic stridor and other signs of respiratory distress. Its diagnosis is established mainly by endoscopic methods, and can be complemented by imaging exams. The therapeutic approach of Pediatric Subglottic Stenosis depends, as well, on its severity and can be addressed by endoscopic techniques, such as Balloon Laryngoplasty, or by open surgery techniques, such as Laryngotracheal Expansion and Reconstruction Surgery or Partial Cricothyroid Resection Surgery.
