Abstract

Ao longo das últimas décadas a obesidade tem-se tornado numa epidemia global e os dados apontam para que na verdade, a maior percentagem de adultos obesos em termos mundiais seja do sexo feminino. As mulheres obesas procuram mais os serviços de saúde para obter ajudar a engravidar, demoram mais tempo e têm menor probabilidade de conseguir alcançar uma gravidez espontânea e têm taxas mais elevadas de interrupções involuntárias de gravidez comparativamente a mulheres com IMC normal. O defeito na fertilidade associado à obesidade tem uma causa multifatorial. Poderá dever-se a uma causa central, associado à diminuição da concentração de LH; estar relacionado com a qualidade do oócito (apresentado estes tamanho e maturação inferior comparativamente aos grupos controlo); estar interligado a alterações na foliculogénese; ou estar associado a alterações na decidualização endometrial. A nível das técnicas de reprodução medicamente assistida, mulheres com excesso de peso ou obesas têm piores resultados após fertilização in vitro (FIV), comparativamente com mulheres de IMC considerado normal. Estudos demonstraram que a perda de peso de pelo menos 10% melhorou as taxas de gravidez e nados vivos através de FIV entre mulheres obesas.

Over the past few decades, obesity has turned into a global epidemic and the data points that the largest percentage of obese adults worldwide is female. Obese women use more frequently health services to get help to conceive, they take longer time and are less likely to achieve a spontaneous pregnancy and have higher abortion rates compared to women with normal BMI. Obesity-related infertility has a multifactorial cause. It may be due to a central cause, with decrease in LH levels; or it may be related to oocyte quality, due to smaller size and decreased oocyte maturation, as well as changes in folliculogenesis; ultimately it may be associated with changes in endometrial decidualization. Obese women have poorer outcomes in assisted reproductive technologies, compared to women with normal BMI. Studies have shown that a weight loss of at least 10% improves pregnancy rates of assisted reproductive technologies.
