Perfis do microbioma oral na cárie e doença periodontal: revisão sistemática de literatura

Abstract

Introdução: Diversos microrganismos habitam a cavidade oral e, em muitos casos, são exclusivos deste nicho, pois desenvolveram uma especificidade requintada para colonização oral. Objectivo: Realizar uma revisão da literatura científica com o intuito de avaliar os perfis do microbioma oral na cárie dentária e na doença periodontal. Metodologia: Pesquisa bibliográfica recorrendo às bases de dados primárias Medline (via PubMed), SciELO e Lilacs , sendo selecionados artigos publicados entre 2000 e 2020, nas línguas inglesa, portuguesa e espanhola. Foram utilizadas as seguintes palavras-chave: “Oral Microbiome”, “Dental caries”, “Periodontal pathogens”, “Biofilm” e “Microbiology”. Os estudos incluídos foram revisões sistemáticas, meta-análises, estudos clínicos controlados e de cohort, todos em humanos, sem restrições em relação à idade, etnia ou género. Resultados: Nesta revisão, foram identificados 253 artigos, nas diferentes bases de dados. A leitura do título e resumo dos artigos permitiu a seleção de 13 publicações com potencial interesse, sendo analisados integralmente. Após aplicação dos critérios de inclusão e exclusão, foram excluídos 8 artigos. A amostra final do estudo foi constituída por 5 publicações. Conclusão: A cárie dentária é uma das doenças crónicas mais comuns mundialmente. A sua patogenicidade está intimamente relacionada ao metabolismo microbioano altamente fermentativo, destacando os streptococcus, particularmente o S. mutans. Já as doenças periodontais possuem bactérias específicas que estão associadas à diferentes formas da doença e severidade da mesma. Os perfis podem ser usados para estabelecer o risco da doença. Porém, mesmo com os avanços tecnológicos, ainda é necessário um maior refinamento dos mesmos.

Introduction: Several microorganisms inhabit the oral cavity and, in many cases, are exclusive to this niche as they have developed an exquisite specificity for oral colonization. Objective: To carry out a review of the scientific literature in order to evaluate the profiles of oral microbiomes in dental caries and periodontal disease. Methodology: Bibliographic search using PubMED, SciELO and Lilacs primary databases, with articles published between 2000 and 2020, in English, Portuguese and Spanish. The following keywords were used: “Oral Microbiome”, “Dental caries”, “Periodontal pathogens”, “Biofilm” and “Microbiology”. The included studies were systematics reviews, meta-analysis, controlled clinical trial and cohort, all in humans, without any restrictions regarding age, sex or ethnicity Conclusions: Dental caries is one of the most common chronic disease worldwide. Its pathogenicity is closely related to highly fermentative microbial metabolism, highlighting streptococcus, particulary S. mutans. Periodontal diseases, on the other hand, have a specific bacteria that are associated with the different types of the disease and their severity. Profiles can be used to establish the risk of the disease. However, even with technological advances, further refinement is still necessary. Results: In this review, 253 articles were identified in different databases. Reading the title and summary of the articles allowed the selection of 13 publications with potential interest, being analyzed in full. After applying the inclusion and exclusion criteria, 8 articles were excluded. The final sample of the study consisted of 5 publications.
