Abstract

A infertilidade afeta 15-20% da população portuguesa em idade reprodutiva, sendo considerada um problema de Saúde Pública. Esta pode ser de causa feminina, masculina, feminina e masculina ou idiopática. No caso da infertilidade de causa feminina, existem, atualmente, vários tratamentos convencionais, que diferem caso a caso, mas que apresentam baixas taxas de sucesso e acarretam alguns riscos, além de poderem representar um desafio psicológico para as mulheres, com consequente descontinuação. Assim, tem-se explorado tratamentos alternativos que, de forma isolada ou conjugada, aumentem as taxas de gravidez em casais inférteis. Alguns tipos de infertilidade feminina, sobretudo a idiopática, são condicionados por mecanismos psicodinâmicos relacionados com o stress, sendo passíveis de resolução por terapêuticas que envolvam a interação mente-corpo, como a hipnoterapia. Contudo, esta abordagem padece de incertezas a nível do seu funcionamento e aplicação prática, por parte dos médicos, o que condiciona a sua atual não inclusão no leque de opções oferecidas às pacientes. A hipnose clínica tem demonstrado ser eficaz no tratamento de alguns casos de infertilidade feminina idiopática, com resultados surpreendentes, e funcionar como coadjuvante de outras terapêuticas, estando incluída nas terapias promissoras na possibilitação das mulheres de engravidar e de conceber uma criança saudável. Contudo, os artigos existentes têm limitações, nomeadamente carecem de critérios bem definidos e controlados, que diminuam a sua heterogeneidade e aumentem a sua reprodutibilidade. Postula-se a realização de mais e melhores estudos, de forma a aumentar a confiança dos médicos em recomendar, referenciar e/ou realizar hipnoterapia, beneficiando as mulheres com um cuidado integrado da sua subfertilidade.

Infertility affects 15-20% of the fertile Portuguese population and is considered a Public Health problem. There are 4 possible causes: feminine, masculine, female and male or idiopathic. In the case of female-caused infertility, there are several conventional treatments, specific to each case but unfortunately with low success rates and accompanied by physical and psychological risks that often lead to poor compliance. For this reason, alternative treatments have been explored which, in an isolated or combined way, increase pregnancy rates in infertile couples. Some types of female infertility, especially idiopathic, are conditioned by psychodynamic mechanisms related to stress, which can be resolved by therapies involving mind-body interaction, such as hypnotherapy. This approach, however, is not sufficiently understood by referring physicians, hindering its inclusion in the options being currently offered to patients. Clinical hypnosis has been shown surprising efficacy in treating some cases of idiopathic female infertility and shows a supporting role to other therapies, aiding women's ability to conceive and give birth to a healthy child. Yet, the existing articles lack well-defined and verified criteria, hindering the needed homogeneity for their experimental reproducibility. It is postulated that more and better studies must be carried out in order to increase physician's confidence in recommending, refer or perform hypnotherapy, benefiting women with integrated care of their subfertility.
