Abstract

A infertilidade é, de uma forma geral, a incapacidade de um casal conceber após 12 meses de relações sexuais. Mundialmente a infertilidade tem aumentado, atingindo 10-15% dos casais e 9-10% a nível nacional. As causas para esta doença são múltiplas, nomeadamente, idade, estilos de vida, distúrbios ovulatórios, endometriose, anormalidades na TF e no útero, causas genéticas, imunológicas e inexplicáveis. Os distúrbios ovulatórios são os que possuem maior incidência, estando divididos pela OMS em 4 classes: 1, 2 (com maior percentagem), 3 e hiperprolatinémia. Para detetar a causa e a terapia mais indicada, realiza-se uma avaliação histórica e física. Para os distúrbios ovulatórios, consoante a sua classe, tem uma terapia específica, para além da mudança do estilo de vida. As várias opções terapêuticas incluem: anti-estrogénios, IA’s, sensibilizadores de insulina, glicocorticóides, gonadotrofinas exógenas, hCG, GnRH pulsátil, DOL, agonista da dopamina, IIU e TRA. Os maiores riscos associados a estas terapêuticas são a GM e SHO. Tem-se realizado vários estudos, a fim de perceber melhor o mecanismo e sugerir novas estratégias terapêuticas, como por exemplo, o uso da combinação BMP-6/gonadotrofinas, para ultrapassar a infertilidade associada ao envelhecimento, e a administração de CNP, como alternativa para a indução de ovulação. Em Portugal, ainda não existe legislação para a PMA. Contudo, para além da medicina seguir padrões de ética médica, existe o CNPMA que controla a atividade de PMA. O Farmacêutico tem um papel ativo na infertilidade feminina, aconselhando, alertando, tranquilizando e, se necessário, o encaminhamento para o médico.

In general infertility is the inability of the couple to conceive after 12 months. Worldwide, infertility is increasing and reaches approximately 10-15% and 9-10% of couples nationwide. The causes for this disease are multiple, such like: age, lifestyles, ovulatory disorders, endometriosis, abnormalities in FT and uterus, genetic, immunological and unexplained causes. The ovulatory disorders are those who have the highest incidence, being divided by the WHO in 4 classes: 1, 2 (with the highest percentage), 3 and hyperprolactinaemic. To detect the cause and the most suitable therapy it is made an historic and physical evaluation. For ovulatory disorders, according to its class, there is a specific therapy, in addition to lifestyle changes. The treatment options include anti-estrogens, aromatase inhibitors, insulin sensitizers, glucocorticoids, exogenous gonadotropins, hCG, GnRH pulsatile, LOD, dopamine agonist, intrauterine insemination and ART. The major risks associated with these therapies are MP and OHS. Several studies have been conducted to better understand the mechanism and suggest new therapeutic strategies, such as, the use of combination BMP-6/gonadotropins to overcome infertility associated with aging and administration of CNP, as an alternative to induce ovulation. In Portugal, there is no legislation to MAP. However, besides the medical care following ethical standards, there is CNPMA which controls the activity of MAP. The pharmacist plays an active role in female infertility, counseling, advising, reassuring and, if necessary, refer to the doctor.
