Presbiacusia : fator de risco para o declínio cognitivo?

Abstract

Tanto a presbiacusia como a demência são entidades clínicas associadas ao envelhecimento, altamente prevalentes nos países desenvolvidos, onde o envelhecimento populacional é um motivo de preocupação cada vez maior. Nos últimos anos, tem surgido cada vez mais evidência da existência de uma associação entre presbiacusia e declínio cognitivo. Embora o mecanismo de associação entre ambos se mantenha pouco claro, inúmeros estudos epidemiológicos evidenciam a presbiacusia como fator de risco modificável para o declínio cognitivo. Esta recente descoberta poderá levar ao desenvolvimento de uma nova abordagem na prevenção da demência. Se realmente se comprovar uma relação de causalidade entre presbiacusia e declínio cognitivo, então a intervenção auditiva com aparelhos auditivos ou implantes cocleares, aplicada precocemente no curso da perda auditiva, poderá atenuar o declínio cognitivo e atrasar o desenvolvimento da demência. O objetivo deste trabalho é apresentar uma revisão da literatura mais recente acerca dos potenciais mecanismos de associação entre declínio auditivo e cognitivo e acerca do impacto da intervenção auditiva na história natural da demência.

Both presbycusis and dementia are age-related clinical entities that are highly prevalent in developed countries, where population aging is a growing concern. Recently, there has been increasing evidence of an association between presbycusis and cognitive decline. Although the mechanism of association between the two remains unclear, numerous epidemiological studies have evidenced presbycusis as a modifiable risk factor for cognitive decline. This recent discovery may lead to the development of a new approach to the prevention of dementia. If a causal relationship between presbycusis and cognitive decline is proven, then hearing intervention with hearing aids or cochlear implants, applied early in the course of hearing loss, may attenuate cognitive decline and delay the development of dementia. The aim of this essay is to present a review of the most recent literature on the potential mechanisms of association between auditory and cognitive decline and the impact of auditory intervention on the natural history of dementia.
