Abstract

As transfusões de sangue constituem uma terapêutica de suporte de vida em doentes com patologias como as Talassemias Major (TM), Drepanocitose e outras anemias crónicas raras. A sobrecarga de ferro transfusional é uma complicação prevísivel da terapia transfusional crónica, com implicações importantes na mortalidade e morbilidade destes doentes. Os métodos diagnósticos, laboratoriais e imagiológicos, em particular, os métodos de imagem por Ressonância Magnética, são ferramentas fulcrais para estimar a quantidade de ferro acumulada no doente e permitir o diagnóstico desta patologia de forma atempada. A terapêutica da sobrecarga de ferro, assenta no uso de agentes quelantes, como a deferoxamina, mas que se associa à má adesão terapêutica. Recentemente, surgiram na prática clínica os novos agentes quelantes orais, a deferiprona e o deferasirox que apresentam perfis de eficácia comparáveis a deferoxamina com melhor tolerância. Este trabalho pretende apresentar a informação clínica necessária para o diagnóstico e terapêutica da sobrecarga de ferro pós transfusional na população pediátrica.

Blood transfusions are a life-saving therapy in patients with pathologies such as Thalassemia Major (TM), Sickle-cell Disease and other rare chronic anemias. Transfusional iron overload is a predictable complication of chronic transfusional therapy with important implications in the mortality and morbidity of this patients. The laboratory and imagiological exams, in particular, Magnetic Resonance image tests are core tools to estimate the amount of iron accumulated in the body and allow the diagnostic of this pathology timely. The therapeuthics of iron overload consist on the use of chelation therapy, such as deferoxamine, but is associated with poor compliace. Recently, arrived in the clinical practice the new oral chelating agents, deferiprone and deferasirox, wich demonstrate efficacy profiles comparable to deferoxamine but with better tolerance. This paper intends to present the necessary clinical information for the diagnostic and therapeutics of transfusional iron overload in the pediatric population.
