Prós e contras da amigdalectomia na amigdalite recorrente em idade pediátrica

Abstract

The present work consists of a revision of the literature regarding the role of tonsillectomy in recurrent tonsillitis in pediatric age. Tonsillectomy has a moderate effect in the reduction of the frequency of sore throats, as well as in aspects concerning quality of life. The effect is only significant in the first year post-surgery and in a restricted part of the population. However, it may also have its risks, whether in the short term or in the long term. Regarding short term complications, hemorrhage and pain are by far the most common ones. In the long term, it can result in changes of the immune system, thus favoring the development of certain diseases. Nevertheless, taking into consideration the scarcity of such works and the possible bias of this observational study, it only serves to alert for this hypothesis and possibly arouse other similar studies in the future. The decision concerning therapy is difficult and it should be done in a multidisciplinary team, incorporating the patients and respective caretakers, who should be informed about the benefits and risks of each option. Each case should be treated individually, considering the severity, influences in the quality of life and expectations.

O presente trabalho consiste numa revisão da literatura quanto ao papel da amigdalectomia na amigdalite recorrente em idade pediátrica. A amigdalectomia proporciona benefício moderado quanto à redução da frequência de episódios de dor de garganta e também em aspetos da qualidade de vida, mais marcado no primeiro ano pós-cirúrgico e para uma população específica. No entanto, também pode apresentar os seus riscos quer a curto prazo, quer a longo prazo. A curto prazo, destacam-se a hemorragia e a dor. A longo prazo, poderá condicionar alterações no sistema imunitário que favoreçam o aparecimento de certas doenças. Contudo, pela escassez de estudos deste género e nos possíveis vieses decorrentes do estudo observacional, serve apenas como um alerta para a existência desta hipótese e possivelmente suscitar a elaboração de estudos semelhantes no futuro. A decisão terapêutica não é linear e deve ser tomada em equipa multidisciplinar, incorporando os doentes e respetivos cuidadores que devem ser informados quanto aos benefícios e riscos de cada opção. Cada caso deve ser abordado individualmente, tendo em conta a gravidade, a influência na qualidade de vida e as expectativas.
