Abstract

A fadiga vocal consiste numa síndrome caraterizada por sintomas de cansaço da voz e sensação de esforço associada à sua utilização, podendo simultaneamente existir dor na região do pescoço. A melhoria da sintomatologia referida pelos doentes após o repouso também é típica. Tal como os distúrbios da voz em geral, a fadiga vocal apresenta uma maior incidência em utilizadores ocupacionais da voz, que tipicamente a utilizam de forma intensa. Surge com maior frequência em mulheres com aumento dos sintomas entre os 20 e os 40 anos. A fisiopatologia é ainda incerta, mas destaca-se uma origem multifatorial no seu desenvolvimento. Estudos de histoquímica demonstraram a predisposição para a ocorrência de fadiga nos músculos laríngeos. Outros fatores como a fuga glótica, o aumento da viscosidade e temperatura das cordas vocais e a diminuição da circulação sanguínea local podem estar envolvidos. Nas mulheres, o sistema respiratório pode também contribuir dada a associação entre menores valores absolutos de FVC (Forced Vital Capacity) e maiores pontuações no VFI (Vocal Fatigue Index). O diagnóstico desta condição baseia-se numa história clínica cuidada, especialmente dirigida aos hábitos de vida e profissão, de modo a escrutinar padrões de utilização de voz propícios ao seu desenvolvimento. O VFI pode ser um auxiliar ao diagnóstico, dada a sua validade científica recentemente comprovada para identificar fidedignamente esta síndrome. Os médicos otorrinolaringologistas devem fazer uma avaliação completa do doente, nuns casos com observação da laringe sem lesões e noutros com patologias como nódulos e paralisias das cordas vocais. A principal abordagem terapêutica consiste na auto consciencialização do problema e na terapia da fala. Além disso devem ser recomendadas alterações do estilo de vida, nomeadamente promover uma correta hidratação ao longo do dia e repouso da voz, que devem ser adequados às necessidades individuais de cada doente.

Vocal fatigue is a syndrome characterized by symptoms of voice tiredness, and effort associated with voice use and sometimes neck pain. The improvement of symptoms referred by patients is also typical. As occurs with general voice disturbances, vocal fatigue has also higher incidence in occupational voice users, which often use it in demanding ways in terms of duration and intensity. It happens more frequently in women with symptoms increasing around the age of 20 to 40 years old. It’s physiopathology is still uncertain, but it likely has a multifactorial development. Histochemistry studies have proved the predisposition of laryngeal muscles towards fatigue. Other factors are glottal air leaking, an increase in vocal fold viscosity and temperature, and decreased blood flow may also be involved. In women, the respiratory system may contribute once there is an association between lower raw values of FVC (Forced Vital Capacity) and higher scores in the VFI (Vocal Fatigue Index). The diagnosis of this condition is based in a carefully made clinical history, specially targeting lifestyle and occupation, so that one can scrutinize voice use patterns associated to vocal fatigue. The VFI may help the diagnosis once it has been scientifically validated to identify this syndrome. Otolaryngologists should manage the patients properly. For instance, in the prescription of ancillary diagnostic tests like a stroboscopic laryngoscopy, because vocal fatigue can appear as associated with other laryngeal pathologies (as vocal fold nodules or vocal fold paralysis). The main therapeutic intervention is voice therapy. Besides that, lifestyle changes should be recommended, like a correct hydration throughout the day, and voice rest that should be tailored to individually needs.
