Abstract

A Vertigem Posicional Paroxística Benigna (VPPB) é a vertigem periférica mais frequente. Caracteriza-se por episódios breves e recorrentes de vertigem e nistagmo, que são desencadeados por mudanças na posição da cabeça em relação à gravidade. É mais comum em mulheres e inicia-se mais frequentemente entre os 50 e os 70 anos. A VPPB é idiopática na maioria dos casos e a canalolitíase é o seu mecanismo fisiopatológico mais comum. As duas variantes clínicas principais são a VPPB do canal semicircular posterior (VPPB-P), que constitui aproximadamente 85% a 95% dos casos de VPPB, e a VPPB do canal semicircular lateral (VPPB-L), que constitui apenas 5% a 15% dos casos. Atualmente, a manobra de Dix-Hallpike e o supine roll test são os exames gold standard no diagnóstico da VPPB-P e da VPPB-L, respectivamente. As manobras de reposicionamento de partículas são o tratamento de eleição da VPPB, sendo a reabilitação vestibular considerada um tratamento adjuvante. O caso clínico apresentado é um exemplo ilustrativo da VPPB recorrente do canal semicircular posterior no adulto jovem, servindo de ponto de partida para uma discussão mais aprofundada acerca dos aspectos clínicos, diagnósticos e terapêuticos da VPPB.

Benign Paroxysmal Positional Vertigo (BPPV) is the most common peripheral vertigo. It is characterized by brief and recurrent attacks of vertigo and nystagmus, which are elicited by changes in head position relative to gravity. It is more frequent in women and starts more often between the 50 and the 70 years old. BPPV is idiopathic in most cases and canalolithiasis is the most common pathophysiological mechanism. The two main clinical variants are posterior semicircular canal BPPV (P-BPPV), which accounts approximately for 85% to 95% of BPPV cases, and lateral semicircular canal BPPV (L-BPPV), which accounts for only 5% to 15%. Currently, the Dix-Hallpike maneuver and the supine roll test are the gold standard tests for the diagnosis of P-BPPV and L-BPPV, respectively. The canalith repositioning maneuvers are the treatment of choice of BPPV and the vestibular rehabilitation is considered an adjuvant treatment. The clinical case presented is an illustrative example of recurrent posterior semicircular BPPV in the young adult, serving as a starting point for a more in-depth discussion about the clinical, diagnostic and therapeutic aspects of BPPV.
