Gastrite autoimune em idade pediátrica

Abstract

Autoimmune gastritis is a rare entity in paediatrics, however it considered to remain underdiagnosed. The diagnosis is very importante due to the unfavorable prognosis and risk of malignancy. It is a chronic inflammatory disease with production of autoantibodies that destroy the parietal cells of the corpus and fundus of the stomach. The resulting achlorhydria induces iron deficiency and later vitamin B12 deficiency. The most frequent clinical manifestation in children is refractory iron deficiency anaemia. Because it’s usually underestimated in clinical-pathological diagnostic investigation, diagnosis is usually delayed, which is done by histological confirmation. Association with other autoimmune diseases is well known. The correlation between autoimmune gastritis and Helicobacter pylori, on the other hand, is controversial. In childhood early diagnosis is important, by the added risk of cancer and because of their long life expectancy. The elaboration of specific guidelines for diagnosis and follow-up of autoimmune gastritis should be emphasized.

A gastrite autoimune é uma entidade rara em idade pediátrica, mas considera-se que permanece subdiagnosticada nos dias de hoje. O seu diagnóstico é muito importante devido ao prognóstico desfavorável e risco de malignidade. É uma doença inflamatória crónica com produção de autoanticorpos que destroem as células parietais do corpo e fundo do estômago. A acloridria resultante leva a uma deficiência de ferro e, posteriormente de vitamina B12. A apresentação clínica mais frequente nas crianças é, de facto, uma anemia por deficiência de ferro refratária. Por ser frequentemente subestimada na investigação diagnóstica clínico-patológica, isso leva a um atraso no diagnóstico, o qual é feito por confirmação histológica. A associação com outras doenças autoimunes é bem conhecida. A relação com o Helicobacter pylori, por outro lado, é controversa. A criação de guidelines específicas de diagnóstico e follow-up para a gastrite autoimune deve ser enfatizada.
