Abstract

The cleft lip and palate is one of the most commonly diagnosed congenital malformation. In about 1 per 1000 births an embryological failure occurs in the fusion of the upper lip and / or the palate giving rise to a cleft lip and / or palate. It is now known that this population has a higher incidence of sensorineural deafness and that about 90% to 100% of these patients have middle ear pathology. Serous otitis media corresponds to the most common middle ear alteration in these patients and is related to the anomalous insertion of the tensor and levator veli palatini muscles leading to a dysfunction in the distal opening of the Eustachian tube, enhancing the development of conductive hearing loss. As a consequence, these patients also have repercussions in other areas that depend directly or indirectly on an adequate auditory function such as language and social and educational development. The controversy associated with the treatment of serous otitis media in children with cleft palate (PF) persists for decades. There are many studies that seek to standardize the techniques offered to these patients. Some authors suggest the placement of ventilation tubes and there is still no agreement with the ideal timing to do so. Others defend more conservative approaches because of the morbidity associated with this surgery, giving primacy to the placement of hearing aids, with good results. About 45% of children with PF suffer from recurrent otorrhea and it is difficult to distinguish whether the etiology of the disease is due to tympanotomy or whether it is related to the otological alterations typical of these patients. In addition to the ear, the cleft palate also affects the respiratory mechanisms, swallowing and sucking ability which can lead to considerable nutritional changes and anemia. It is widely accepted that these children should be closely monitored by an Otorhinolaryngologist throughout their life.

A fenda labiopalatina é uma das malformações congénitas mais frequentes. Em cerca de 1 por cada 1000 nascimentos ocorre uma falha embriológica na fusão do lábio superior e/ou do palato dando origem a uma fenda labial e/ou palatina. Sabe-se atualmente que esta população tem uma maior incidência de surdez neurossensorial e que cerca de 90% a 100% destes doentes têm patologia do ouvido médio. A otite média serosa corresponde à alteração do ouvido médio mais comum e está relacionada com a inserção anómala dos músculos tensor e elevador do véu do palato originando uma disfunção na abertura distal da trompa de Eustáquio, potenciando o desenvolvimento de hipoacusia de condução. Por consequência, estes doentes têm também repercussões noutras áreas que dependem direta ou indiretamente de uma função auditiva adequada como a linguagem e o desenvolvimento social e educacional. A controvérsia associada ao tratamento da otite média serosa em crianças com fenda palatina (FP) persiste há décadas. Muitos são os estudos que procuram uniformizar as técnicas oferecidas a estes doentes. Alguns autores sugerem a colocação de tubos de ventilação não havendo ainda concordância com o timing ideal para o fazer. Outros defendem abordagens mais conservadoras devido à morbilidade associada a esta cirurgia dando primazia à colocação de próteses auditivas, com obtenção de bons resultados. Cerca de 45% das crianças com FP sofrem de otorreia recorrente, sendo difícil distinguir se a etiologia da mesma é pela timpanotomia ou se está relacionada com as alterações otológicas próprias destes doentes. Para além do ouvido, a fenda palatina afeta também os mecanismos respiratórios, a deglutição e a capacidade de sucção podendo levar alterações nutricionais consideráveis e anemia. É amplamente aceite que estas crianças devem ser alvo de uma estreita vigilância por um otorrinolaringologista ao longo da sua vida.
