Potencial evocado miogénico vestibular : importância clínica

Abstract

Vestibular-evoked myogenic potentials (VEMPs) are short latency potentials that result from the muscular response triggered by the activation of the vestibular receptors, stimulated by sound or vibration. This muscular response, as measured by the electromyogram, is observed in the sternocleidomastoid muscle, in the cervical VEMP (cVEMP) and in the extraocular inferior oblique muscle, in the ocular VEMP (oVEMP). With this, the two types of VEMPs, the cervical and the ocular, will be discussed. The organs involved in these reflexes are essentially the saccule and the utricle, organs which contain the otoliths. Thus, this test is used in the evaluation of the vestibular system. This article aims to approach in a concise and easily way, the discussion of the two types of vestibular-evoked myogenic potentials, their anatomical basis, properties, methodology, measurement and clinical application in the different vestibular pathologies.

Os potenciais evocados miogénicos vestibulares (VEMPs) são potenciais de curta latência que resultam da resposta muscular desencadeada pela ativação dos recetores vestibulares, estimulados por via sonora ou vibratória. Esta resposta muscular, medida pelo eletromiograma, é observada no músculo esternocleidomastoideu, no VEMP cervical (cVEMP) e no músculo extraocular oblíquo inferior, no VEMP ocular (oVEMP). Com isto, vão ser discutidos os dois tipos de VEMPs, o cervical e o ocular. Os órgãos implicados nestes reflexos são, essencialmente, o sáculo e o utrículo, órgãos que contêm os otólitos. Assim, este teste é usado na avaliação do sistema vestibular. Este artigo visa abordar de forma concisa e de fácil compreensão a discussão dos dois tipos de potenciais evocados miogénicos vestibulares, a sua base anatómica, propriedades, metodologia, medição e aplicação clínica nas diferentes patologias vestibulares.
