Abstract

Introdução: neste trabalho é feita uma breve referência à anatomia dos seios perinasais, um conjunto de cavidades pneumáticas revestidas por mucosa, localizados no interior de alguns ossos da face, no sentido de compreender a estreita relação entre o seio maxilar, a arcada dentária superior, e a cavidade oral para ser abordada em detalhe a sinusite maxilar de etiologia dentária. Sinusite define-se como uma inflamação da mucosa dos seios perinasais cujo diagnóstico assenta na colheita de anamnese e de exane objectivo minucioso, podendo dividir-se, consoante o tempo de evolução do quadro, em aguda (até 4 semanas) e crónica (superior a 4 semanas), e apresentar diversas causas, abordagens terapêuticas e prognósticos. São também abordadas várias prevalências descritas na literatura e os microrganismos mais comumente responsáveis pela sinusite odontogénica, assim como os seus fatores de risco. Caso clínico: o caso exposto refere-se a um doente do sexo masculino, 43 anos, caucasiano que recorreu a uma instituição hospitalar particular por queixas sugestivas de sinusite maxilar aguda e com falência no tratamento inicial da mesma, tendo sido submetido a TC dos seios perinasais que confirmou o diagnóstico. Por recorrência do quadro e falência no tratamento médico, o doente terá sido proposto e submetido a intervenção cirúrgica para resolução do quadro, tendo apresentado uma boa evolução. Discussão: Enquadramento da sintomatologia do doente com o que é descrito na bibliografia, discussão dos melhores métodos complementares de diagnóstico para esta entidade clínica e procedimentos cirúrgicos possíveis para tratamento de patologia refratária a terapêutica médica, recorrente ou complicada.

Introduction: in this case study a brief reference to the anatomy of the perineal sinuses, a set of pneumatic cavities lined by mucosa, located inside some bones of the face, in order to understand the close relationship between the maxillary sinus, upper dental arcade, and oral cavity, so that maxillary sinusitis of dental aetiology can be then discussed in detail. Sinusitis is defined as an inflammation of the nasal sinus mucosa, whose diagnosis is made in throw detailed anamnesis and physical examination. According to evolution time, it can be divided in acute (up to 4 weeks) and chronic (more than 4 weeks), and present various causes, therapeutic guidelines and prognoses. Several prevalences described in the literature, the microorganisms most commonly responsible for odontogenic sinusitis, as well as their risk factors, are also addressed. Case report: the presented case concerns a 43-year-old caucasian male patient who went to a private hospital with complaints suggestive of acute maxillary sinusitis forwhich he got medicated with no success, having then undergone CT which confirmed the diagnosis. Due to recurrence of his condition and failure of medical therapy, the patient was proposed for and underwent surgical surgery to resolve the problem, with success. Discussion: enfolds framing of the patient's symptomatology with what is described in the bibliography, discussion of the best auxiliary diagnostic methods for this clinical entity and possible surgical procedures for the treatment of refractory, recurrent or complicated disease.
