Autoimunidade multiorgânica com envolvimento hepático

Abstract

As doenças autoimunes afetam 5% da população mundial. Estes doentes têm maior suscetibilidade para desenvolver patologias autoimunes adicionais. As entidades conhecidas associando várias patologias autoimunes são escassas. A síndrome múltipla autoimune é a combinação de três ou mais doenças autoimunes num mesmo indivíduo. Apresenta-se um caso de um indivíduo de sexo masculino que, no primeiro ano de vida desenvolveu doença celíaca mas que manteve a ingestão de glúten por onze anos. Mais tarde, desenvolveu outras patologias autoimunes. Ainda em idade pediátrica apresentou psoríase ligeira e hepatite autoimune. Já em idade adulta, manifestou miocardiopatia dilatada, vitiligo, colite microscópica e surdez autoimune. Atualmente, com terapêutica imunossupressora, apresenta-se pouco sintomático e sem diminuição significativa da qualidade de vida.

Autoimmune diseases affect 5% of the world’s population. These patients have an increased risk to develop other autoimmune disorders. There are few syndromes which combines different autoimmune diseases. Multiple autoimmune syndrome is the combination of three or more of these pathologies in the same person. This case is about a man who developed coeliac disease, as a child, but ingested gluten for eleven years. Later, he presented psoriasis and autoimmune hepatitis. As an adult he developed dilated cardiomyopathy, vitiligo, microscopic colitis, and autoimmune inner ear loss. He is currently undergoing immunosuppressive therapy and has few symptoms which don’t affect his quality of life.
