Abstract

A fratura da extremidade proximal do úmero é a quarta fratura osteoporótica mais comum, esperando-se ainda um aumento da sua incidência nos próximos anos face ao envelhecimento da população. A idade do paciente foi identificada como fator de risco, o que torna a população geriátrica mais suscetível a fraturas complicadas resultantes de traumatismos de baixa energia, com maior cominução das tuberosidades e descoaptação. Nas fraturas complicadas de três e quatro partes, com necessidade de intervenção cirúrgica, as opções terapêuticas mais utilizadas são a redução e osteossíntese, a hemiartroplastia (HA) e a artroplastia total do ombro com prótese inversa (ATOPI). Devido à elevada incidência de complicações pós-cirúrgicas e, porque a longevidade da prótese não está ainda bem definida, a ATOPI propõe-se ser a artroplastia mais indicada no idoso, apresentando maiores benefícios face à HA, na medida em que não é tão dependente da manutenção da função das tuberosidades do úmero nem da coifa dos rotadores (CR). Os resultados funcionais da ATOPI são mais previsíveis do que a HA, sendo esta última mais eficaz no controlo da dor pós-fratura, embora não produza resultados funcionais tão satisfatórios. A complicação mais comum após ATOPI é a instabilidade dos componentes protésicos, associada a notching escapular, rotura da CR, fratura peri-protésica e lesão do nervo axilar. A ATOPI revela-se vantajosa no tratamento de fraturas complexas da extremidade proximal do úmero no idoso com bons outcomes funcionais na maioria dos estudos, havendo no entanto, necessidade de uma investigação mais detalhada nesta área, direcionada para os outcomes mais importantes no idoso: independência e qualidade de vida.

The proximal humerus fracture is the fourth most common among osteoporotic fractures, and its incidence is expected to rise in the next few years, alongside the population’s aging. Patients’ age has been established as a risk factor, placing the geriatric population in a more susceptible position for complicated fractures resulting from low energy trauma, with greater tuberosity comminution and displacement. In 3 and 4 part complicated fractures in need of a surgical approach, the most used approaches are reduction and osteosynthesis, hemiarthroplasty (HA) and reverse shoulder arthroplasty (RSA). Due to the high rate of post-operative complications and not yet established longevity, RSA seems to be a more suitable arthroplasty for the elderly patient, theoretically presenting with greater benefits comparing with HA, as it’s not as dependent on tuberosity and rotator cuff function levels. RSA presents more predictable functional outcomes than HA, that in its turn provides better pain relief but less satisfactory functional outcomes. The most frequent complications after RSA are component instability, often associated with scapular notching, rotator cuff rupture, periprosthetic fracture and axillary nerve lesion. RSA appear to be advantageous in the treatment of complex proximal humeral fractures presenting good functional outcomes in most studies, with further need of detailed investigation in this field, directed towards the most important factors to the elderly patient, like independence and quality of life.
