Pacientes geriátricos na clínica de medicina e cirurgia oral da FMDUL

Abstract

Introdução: Com os avanços da medicina e dos cuidados de saúde pública das últimas décadas a população idosa tem vindo a aumentar substancialmente. Este grupo etário pelas suas alterações fisiológicas, co-morbilidade, polimedicação, uso de próteses e diminuição da quantidade e qualidade da higiene oral está mais suscetível a doenças orais. O médico dentista tem, assim, um papel fundamental no diagnóstico e tratamento destes pacientes. Objetivos: Caraterizar a população geriátrica com algum tipo de patologia oral presente e que se dirigiu à consulta de CCMO I e CCMO II da FMDUL de 2012 a 2019, bem como os procedimentos e tratamentos efetuados. Materiais e Métodos: Realizou-se um estudo epidemiológico observacional e descritivo com uma componente analítica de uma série de casos de pacientes geriátricos que frequentaram a consulta de CCMO I e CCMO II da FMDUL entre 2012 e 2019. Foram incluídos 181 pacientes e registados os dados referentes aos mesmos. A análise estatística foi realizada com recurso ao software SPSS versão 25 (IBM, Armkong, NY, USA) Resultados: Os pacientes são maioritariamente do género feminino. A média de idades foi cerca de 73 anos e mais de 90% da população era polimedicada. Realizou-se biópsia em 33,5% da amostra. As patologias mais frequentes foram a hiperplasia fibrosa focal, o BMS, o LPO e a hiperqueratose friccional. Verificou-se diferença estatisticamente significativa apenas em relação ao BMS o qual se mostrou mais prevalente no género feminino. Discussão: As lesões traumáticas foram as mais comuns, tal como referido na literatura em relação a esta população. O LPO foi a doença auto-imune mais prevalente. Conclusão: O aumento da população geriátrica e a sua suscetibilidade a doenças orais são uma realidade. O Médico Dentista desempenha um papel fundamental no diagnóstico e tratamento na área da medicina e patologia orais

Introduction: With the improvements in medicine and public oral health in the last decades the elderly population has been growing substantially. This specific age group because of its physiologic changes, morbility, polimedication and use of prosthetics is more susceptible to oral diseases. That being said, dentists have a very important role in the diagnosis and treatment of these patients. Purpose: To characterize a range of geriatric patients with oral pathologies as well as the procedures and treatments preformed. Study design: An epidemiological observation study, with geriatric patients that attended to the appointment of Oral Surgery and Medicine Clinic, at Faculdede de Medicina Dentária de Lisboa was conducted. There were 181 patients included and their data was recorded. The data analysis was performed using the software SPSS versão 25 (IBM, Armkong, NY, USA) Results: The patients were mainly woman. The mean age was about 73 years old and more then 90% were polimedicated. In 33,5% of the sample biopsies were conducted, The most common oral pathologies observed were irritative hyperplasia, burning mouth syndrome, oral lichen planus and frictional keratosis. There was a relationship with statistical significance regarding BMS and gender with it being more common in women. Discussion: The traumatic lesions were the most prevalent pathologies, as suggested in the literature. The oral lichen planus was the most common auto-immune disease. Conclusion: The constant growth of the elderly population and its susceptibility to oral diseases are a reality. Dentists have an important role in the diagnosis and treatment when it comes to cases related to oral medicine and oral pathology.
