Abstract

A infeção por MP (Mycoplasma pneumoniae) é uma causa comum de pneumonia adquirida na comunidade em idade pediátrica, podendo associar-se a doença extrapulmonar. A manifestação dermatológica é frequente em crianças e pode traduzir-se por EM (Eritema multiforme). Apresenta-se o caso clínico de uma adolescente de 13 anos, previamente saudável, admitida na urgência por febre e tosse, acompanhada posteriormente de hiperémia conjuntival e exantema vesicular com lesões em alvo. Ao exame objetivo inicial salientava-se envolvimento mucocutâneo com hiperémia ocular bilateral, vesículas na orofaringe e mucosa labial nacarada, assim como exantema maculo-papulo-vesicular atingindo áreas da face, tronco e coxa. Evolução clínica compatível com Eritema multiforme major. Durante o internamento, colocada a hipótese diagnóstica de pneumonia, com fervores crepitantes na auscultação, e radiografia torácica com infiltrado intersticial bilateral e hipotransparência na base direita. No seguimento da avaliação clínica e laboratorial, com serologia positiva para Mycoplasma pneumoniae, foi medicada com claritromicina, revelando evolução clínica favorável, com resolução das lesões cutâneas. O Eritema multiforme resulta de uma reação de hipersensilidade desencadeada, entre outros, por fármacos ou infeções a vários agentes. Apesar da associação ao Mycoplasma pneumoniae ser menos frequente, deve ser considerada na presença de sintomatologia respiratória.

Infection by Mycoplasma pneumoniae is a common cause of community-acquired pneumonia in children and may be associated with extrapulmonary disease. The dermatologic manifestation is common in children and can be manifested by erythema multiforme. We present the case of a 13 year old girl, previously healthy, admitted by fever and cough, later accompanied by conjunctival hyperemia and vesicular rash with target lesions. On physical examination there was mucocutaneous involvement with bilateral ocular hyperemia, vesicles in the oropharynx and nacreous labial mucosa, and maculo-papular-vesicular rash reaching areas of the face, torso and thigh. Clinical course consistent with erythema multiforme major. During hospitalization, it became likely the diagnosis of pneumonia, with crackles on auscultation and chest x-ray with bilateral interstitial infiltrate and hypotransparency on the right basis. Following the clinical and laboratory evaluation, with positive serology for Mycoplasma pneumoniae, the patient was treated with clarithromycin, revealing favorable clinical evolution, with resolution of skin lesions. Erythema multiforme result of a hypersensitivity reaction initiated, among others, drugs or infections by various agents. Despite the association with Mycoplasma pneumoniae is less frequent, it should be considered in the presence of respiratory symptoms.
