Ortodontia intercetiva na correção da mordida cruzada anterior: a propósito de um caso clínico

Abstract

Introdução: A mordida cruzada anterior é uma maloclusão caracterizada pela presença clínica de overjet negativo, com alteração da relação labiolingual de um ou mais dentes dos maxilares superiores e inferiores. A prevalência na população varia entre os 2,2 e os 19%, e depende de fatores como a idade e grupo étnico do paciente. De acordo com a sua etiologia esquelética, dentária ou funcional podemos classificar a mordida em: classe III esquelética, classe III dentária e classe III funcional ou pseudo classe III. Objetivos: Perceber a importância do correto diagnóstico diferencial para a escolha do tratamento intercetivo mais indicado, na correção da mordida cruzada anterior e avaliar o sucesso terapêutico num adulto da aplicação de um aparelho removível intercetivo, neste tipo de maloclusão. Materiais e métodos: Realizou-se uma pesquisa bibliográfica na base de dados PubMed com artigos datados entre 1997 e 2017. A pesquisa foi filtrada por: análises sistemáticas, ensaios clínicos randomizados e casos clínicos. De 480 artigos pesquisados, foram selecionados, consoante o formato disponível em inglês, título e resumo, 31 artigos científicos. Foram, ainda, consultados 7 livros. Palavras-chave: diagnóstico diferencial da classe III; correção da mordida cruzada anterior; aparelhos intercetivos; crianças; adultos. Resultados: O diagnóstico diferencial da classe III revela-se importante na decisão terapêutica mais indicada, seja ela não invasiva (ortodôntica ou ortopédica) ou invasiva (cirúrgica/ortognática). Na abordagem intercetiva recorre-se a dispositivos ortopédicos – Face Mask, Chin Cup e Modified Progenic, e dispositivos funcionais – Balter´s Bionator III, Activator III, Reverse Twin Block e Frankel III Regulator. No caso clínico descrito verificaram-se resultados positivos, mas não ideais, uma vez que a correção da mordida foi alcançada até à posição de topo a topo. Conclusões: Principalmente em adultos, o tratamento de maloclusões deve ter em conta objetivos atingíveis, de modo a eliminar as queixas do paciente e promovendo a estética e função intermaxilar.

Introduction: Anterior crossbite is a malocclusion that refers to a abnormal relationship, of one or more teeth, between maxilla and mandible, and it is clinically represented by a reverse overjet. The malocclusion prevalence is related with age and ethnicity and varies between 2,2 and 19%. According to anterior crossbite etiology, we can classify as: skeletal class III; dental class III and pseudo class III/ functional class III. Objectives: To understand the differential diagnosis importance on the correct and interceptive treatment choice, to achieve an anterior crossbite correction; as well as to perceive the effects of an removable interceptive appliance on an adult patient, with class III malocclusion. Materials and methods: An article research, on PubMed, was made since 1997 until 2017. The selection criteria included on this thesis: systematic reviews, randomized clinical trials and cases reports. Relevant journals and reference lists of included studies were manually searched by language (English), title and abstract. In total of 480 searched articles, 31 articles were collected and 7 electronic books were consulted. Keywords: class III differential diagnosis; correction of anterior crossbite; interceptive appliances; children; adults. Results: The class III differential diagnosis has a major relevance on the treatment options that are better for the patient; they could be more conservative (orthodontic or orthopedic) or more invasive options (orthognathic surgery). About interceptive treatment options there are different kinds of appliances: orthopedic appliances – Face Mask, Chin Cup e Modified Progenic and functional appliances – Balter´s Bionator III, Activator III, Reverse Twin Block e Frankel III Regulator. On the case report, the clinical success was not complete since the malocclusion correction was just made until the edge to edge position. Conclusions: Mainly in adults, the malocclusion treatment should have in mind achievable goals, eliminating the patient complains and promoting esthetics and function.
