Abstract

A hemorragia digestiva alta é uma condição comum num Serviço de Urgência Hospitalar, com morbilidade e mortalidade associadas significativas, várias etiologias possíveis e prognósticos diferentes. Os métodos endoscópicos são primeira linha na marcha diagnóstica e terapêutica da hemorragia digestiva. Porém, existem casos em que após a sua utilização, não é possível a definição etiológica sendo classificada como etiologia obscura. A lesão de Dieulafoy é uma causa rara de hemorragia digestiva, diagnosticada na sua maioria por endoscopia, e por vezes após várias endoscopias com difícil diagnóstico na abordagem inicial. Tal facto pode-se dever ao seu carácter intermitente e com apresentação semelhante a outras etiologias, com consequente impossibilidade de identificação e resolução endoscópica imediatas. Este trabalho pretende enfatizar a necessidade de uma marcha diagnóstica sistemática e diferencial na hemorragia digestiva, cujo diagnóstico etiológico e terapêutica nem sempre são imediatos e definitivos com crucial impacto da abordagem conjunta com outras áreas de intervenção, nomeadamente com a Radiologia.

Upper gastrointestinal bleeding is a common condition in a Hospital Emergency Room, with significant associated morbidity and mortality, many possible etiologies and different prognosis. Endoscopic methods are the first line in the diagnostic and therapeutic course of gastrointestinal bleeding. However, there are cases in which, after its use, the etiologic definition can't be obtained, being classified as obscure etiology. The Dieulafoy's lesion is a rare cause of gastrointestinal bleeding, diagnosed mostly by endoscopy, and sometimes after several endoscopies with difficult diagnosis in the initial approach. This may be due to its intermittent nature and similar presentation to other etiologies, with consequent impossibility of immediate identification and endoscopic resolution. This work intends to emphasize the need for a systematic and differential diagnostic process in digestive bleeding, whose etiological and therapeutic diagnosis are not always immediate and definitive, with a crucial impact of the joint approach with other areas of intervention, namely Radiology.
