Abstract

O envelhecimento da população mundial e o aumento da prevalência da apneia do sono com a idade tornam imperativo compreender as especificidades desta doença no idoso. Não parece haver um mecanismo específico que justifique o elevado número de casos de apneia do sono na velhice. A apresentação clínica deste distúrbio respiratório do sono no idoso é frequentemente atípica, passando por noctúria, compromisso cognitivo e repercussões nas actividades de vida diária, com menor sonolência e impacto na qualidade de vida. A apneia do sono tem consequências cardiovasculares, cognitivas e psiquiátricas, aparentemente com menor efeito na morbimortalidade cardiovascular comparativamente a doentes mais novos. O diagnóstico de apneia do sono no idoso deve ser feito com polissonografia e a terapêutica recomendada passa por alterações comportamentais e ventiloterapia. O tratamento com CPAP (continuous positive airway pressure) tem vários efeitos benéficos e diminui a utilização dos cuidados de saúde e custos inerentes. Desta forma, é fundamental aumentar o conhecimento sobre a apneia do sono no idoso a fim de criar guidelines que tenham em consideração as particularidades desta população.

World population ageing and sleep apnea increasing prevalence with age make it imperative to understand this disease’s specificities in the elderly. It does not seem to be a specific mechanism that justifies the high number of sleep apnea cases in old age. This respiratory disorder’s clinic presentation is frequently atypical in elderly, including nocturia, cognitive impairment and daily life activities repercussions, with less somnolence and impact on quality of life. Sleep apnea has cardiovascular, cognitive and psychiatric consequences, apparently with less effect on cardiovascular morbidity and mortality comparatively to younger patients. Sleep apnea in the elderly should be diagnosed with polysomnography and the recommended therapy includes behavioral changes and ventilotherapy. CPAP (continuous positive airway pressure) treatment has multiple beneficial effects and decreases health care utilization and inherent costs. So it is fundamental to increase the knowledge about sleep apnea in the elderly in order to create guidelines which take into consideration this population’s particularities.
