Abstract

A infertilidade feminina é considerada pela Organização Mundial de Saúde (OMS) um problema de saúde pública. A prevalência de infertilidade nos países Europeus é cerca de 14%, afetando um em cada sete casais. Os tratamentos disponíveis e a sua efetividade têm repercussões significativas nos casais, deste modo o objetivo desta revisão consiste na identificação dos meios de diagnóstico e abordagens terapêuticas disponíveis, bem como das perspetivas futuras que permitem aumentar a comodidade, flexibilidade e efetividade dos tratamentos atuais. A pesquisa foi efetuada entre Março e Setembro de 2013, através de bases de dados eletrónicas: PubMED, B-on, Elsevier, e adicionalmente através de guidelines publicadas pelo “National Institute for Health and Care Excellence” (NICE), e pelas Sociedades de Medicina Reprodutiva Europeia (ESHRE) e Americana (ARSM). A abordagem terapêutica na infertilidade pode ser dividida em três grupos: Farmacológica, Cirúrgica e Tecnologias de Reprodução Assistida. De modo a aumentar a comodidade e flexibilidade relativamente à terapêutica farmacológica atual, tem sido objeto de investigação o desenvolvimento de fármacos com biodisponibilidade oral. O citrato de clomifeno (CC) foi durante muito tempo o único fármaco administrado por via oral, no entanto devido à existência de casos de resistência, os inibidores da aromatase (IA) passaram a constituir uma alternativa. Encontram-se também em estudo moléculas de baixo peso molecular, com biodisponibilidade oral e propriedades agonistas dos recetores da FSH e LH, como alternativa às gonadotrofinas injetáveis. Atualmente, o CC permanece como terapêutica de primeira linha, são necessários mais estudos de eficácia e segurança na avaliação dos IA e mais estudos no desenvolvimento dos agonistas dos FSHr e LHr. As ART não são consideradas primeira linha, dada a existência de métodos menos invasivos.

Female infertility is considered by the World Health Organization (WHO) a public health problem. Infertility’s prevalence in Europe countries is about 14%, one in seven couples. Available treatments and their effectiveness have a significant impact on couples, thus the aim of this review is the identification of diagnostic and therapeutic approaches, and future prospects that increase flexibility, compliance and effectiveness of current treatments. The research was carried out between March and September 2013, through electronic databases: PubMED, B-on, Elsevier, and guidelines published by the "National Institute for Health and Care Excellence" (NICE), European (ESHRE) and American’s (ARSM) Reproductive Medicine Societies. Available treatments can be divided into three groups: Pharmacologic, Surgical and Assisted Reproductive Technologies. In order to increase current pharmacological treatment’s compliance, the research has been conducted to find drugs with oral bioavailability. Clomiphene citrate (CC) has long been the only orally administered drug, however due to reported resistance cases, the aromatase inhibitor (AI) takes place as an alternative. Current development has focus on molecules with low molecular weight with oral bioavailability and FSH/LH receptor agonist properties, as an alternative to injectable gonadotropins. Today, CC stands as first-line therapy, more studies are needed on the efficacy and safety evaluation of AI and on the development of FSHr and LHr agonists. The ART are not considered first therapy line, given the existence of less invasive methods.
