Abstract

Objective: Objective: To characterize the demographics of patients with acute cholecystitis and analyze clinical and therapeutical decision taking, according to the patients’ characteristics. Material and Methods: Retrospective analyses of patients processes that turned to the Emergency Department of CHLN between year 2012 and 2015 with acute cholecystitis diagnosed. Results: From the 313 patients diagnosed with acute cholecystitis, with an average age of 63.9 years, 71.4% presented with a smaller or equal to 72 hours evolution, and from these, 63.1% were treated in a conservative way and 32.4% were submitted to an early cholecystectomy. From the symptomatic patients with more than 72 hours, 64.8% were treated in a conservative way and 29.5% were submitted to an early cholecystectomy. Internal patients with this diagnosis were submitted to hospitalization with antibiotic therapy in 63.6% of cases. Conclusions: Despite the available scientific evidence favor early cholecystectomy, particularly in the first 72 hours, we found out that although a significant percentage of patients turned to the urgency during this period, early cholecystectomy was not submitted to any of them.

Objectivo: caracterizar a demografia dos doentes com diagnóstico de colecistite aguda e as tomadas de decisão clínica e terapêutica face às características dos doentes. Material e Métodos: estudo retrospectivo com avaliação dos processos de doentes com diagnóstico de colecistite aguda que recorreram ao Serviço de Urgência do CHLN entre 2012 e 2015. Resultados: 313 doentes com diagnóstico de colecistite aguda, com média de idade de 63,9 anos. 71,4% apresentava-se com uma evolução menor ou igual a 72 horas, sendo que destes, 63,1% foram tratados conservadoramente e 32,4% foram submetidos a colecistectomia precoce. Dos doentes sintomáticos com mais de 72 horas, 64,8% foram tratados conservadoramente e 29,5% submetidos a colecistectomia precoce. Os doentes internados com este diagnóstico foram submetidos a internamento com antibioterapia em 63,6% dos casos. Conclusões: Apesar da evidência científica disponível favorecer a colecistectomia precoce, particularmente nas primeiras 72 horas verificamos que, apesar de uma percentagem significativa dos doentes recorrer à urgência neste intervalo de tempo, não foi submetida a colecistectomia precoce.
