Bruxismo em pessoas com paralisia cerebral

Abstract

Objetivo: determinar a prevalência de bruxismo geral (BG), bruxismo do sono (BS), bruxismo de vigília (BV) e bruxismo misto (BM) em indivíduos com paralisia cerebral (PC), avaliando os diversos fatores e comorbilidades associadas. Materiais e métodos: selecionaram-se seis instituições para pessoas com necessidades especiais, sendo a amostra constituída por 110 indivíduos com PC. A recolha de dados foi realizada através da consulta do processo clínico do utente existente na instituição, pela observação clínica e pelos dados fornecidos pelos pais/tutores legais ou pelo próprio. A observação clínica teve por base os critérios de pesquisa para as disfunções temporo mandibulares. Para o diagnóstico de BS utilizaram-se os critérios de diagnóstico propostos pela Academia Americana de Medicina do Sono (AAMS), em 2001, e para a classificação de espasticidade muscular, a escala modificada de Asworth. Resultados: a população estudada era maioritariamente do género masculino (52,7%), com uma média de idades de 42,9±13,61. No geral, 74,5% dos indivíduos apresentavam bruxismo. Foi encontrada uma associação entre a presença de movimentos involuntários nos membros superiores e o BV (p=0,034). Quando avaliada a presença de facetas de desgaste verificou-se uma associação significativa com todos os tipos de bruxismo estudados. Relativamente ao brilho das facetas de desgaste, verificou-se uma associação com o BM e o BG (p=0,002 e p=0,001, respetivamente). Observou-se uma associação entre o BM e a classificação de espasticidade (p=0,047). Por último, através da regressão logística constatou-se que o brilho é significativo na previsão do BM (p=0,022). Conclusões: com base na elevada prevalência de bruxismo na amostra, verificou-se uma necessidade urgente para a implementação de opções de tratamento para esta patologia em pessoas com PC. São necessários mais estudos, com protocolos de diagnóstico padronizados e amostras representativas para avaliar os fatores que influenciam a presença dos vários tipos de bruxismo.

PURPOSE: The aim of this study was to determine the prevalence of bruxism in general (GB), sleep bruxism (SB), awake bruxism (AB) and mixed bruxism (MB) in individuals with cerebral palsy (CP) evaluating the various factors and associated comorbidities. MATERIALS AND METHODS: One hundred and ten individuals diagnosed with cerebral palsy were selected from six institutions for people with special needs. Data collection was done through the consultation of the patient's clinical file in the institution, clinical observation and from data provided by the parents / legal guardians or by the patient himself. Clinical observation was based on the clinical diagnosed criteria for temporomandibular disorders. The diagnostic criteria proposed by the American Academy of Sleep Medicine (AASM) in 2001, was used for the diagnosis of SB, and the modified scale of Asworth for the classification of muscular spasticity. RESULTS: In this study the majority of individuals were male (52,7%) with a mean age of 42,9 ± 13,61. A total of 61.5% resided or attended the institution for more than 15 years. Bruxism was present in 74,5% of individuals. An association was found between the presence of involuntary movements in the upper limbs and awake bruxism (p = 0.034). A significant association between wear facets and all types of bruxism was also found. Regarding wear facets brightness an association was found with MB and GB (p=0,002 and p=0,001, respectively). An association between MB and spasticity was found (p=0,047). Finally, by analyzing a multiple logistic regression model, it was found that brightness is significant in the prediction of MB (p=0,022). CONCLUSION: Based on the high prevalence of bruxism found there is an urgent need for treatment options in people with CP. More studies are needed with standardized diagnostic protocols and representative samples to evaluate the factors that influence the presence of various types of bruxism.
