Abstract

A espondilolistese é uma doença que afeta a população pediátrica, com uma incidência de cerca de 6%. Consiste no deslizamento de uma vértebra sobre outra subjacente e pode trazer complicações neurológicas importantes. A sua causa é multifatorial e o tratamento é um assunto bastante debatido mas nada consensual. Na avaliação do doente existem diversas classificações, sendo que recentemente o “Spinal Deformity Study Group” desenvolveu uma escala que tem em conta o grau de deslizamento, de acordo com a classificação de Meyerding, o equilíbrio espino-pélvico e a incidência pélvica, que permite ponderar qual a melhor abordagem para cada doente. No caso da espondilolistese de alto grau em idade pediátrica, com um grau de deslizamento acima de 50%, o tratamento cirúrgico pode estar indicado mesmo em doentes assintomáticos. O cirurgião poderá então optar por realizar uma artrodeseinsitu, sem restauração da anatomia sagital, ou a redução da vértebra deslizante com posterior fusão, com reconhecidos benefícios no que toca ao equilíbrio espino-pélvico e na taxa de fusão. No entanto, esta abordagem nem sempre é seguida dada a complexidade do procedimento. A instrumentação poderá ser utilizada quando existe um risco de progressão do defeito, embora pareça não resultar numa melhoria da sintomatologia. Apesar das várias complicações que podem advir do procedimento cirúrgico, os resultados funcionais são geralmente favoráveis, pelo que a cirurgia mantém-se como a abordagem mais correta do doente pediátrico com espondilolistese de alto grau.

Spondylolisthesis is a disease that is seen in the pediatric population, with an incidence of about 6%. Consists on a slippage of one vertebra over an underlying one and can bring important neurological complications. With a multifactorial cause, the treatment is one of the most debatable issues, remaining a matter of significant discordance. In the evaluation of the patient there are several classifications, having the “Spinal Deformity Study Group” recently developed a scale that incorporates the degree of slip, according to Meyerding’s classification, the spinopelvic and the pelvic incidence, which helps one to consider the best approach to each patient. Regarding high-grade pediatric spondylolisthesis, with a degree of slip over 50%, surgery may be advised even in asymptomatic patients. The surgeon may have to choose between an arthrodesis in situ, without restoration of sagital anatomy, or the reduction of the slipped vertebra with posterior fusion. Nevertheless, reduction is not always done for its complexity. Instrumentation can be used when there is a risk of the slip progression, even though it does not seem to lessen symptoms. Although many complications may arise from surgical procedure, functional results are generally favorable. Despite all controversy, surgery appears to be the best approach to the high-grade pediatric spondylolisthesis patient.
