Abstract

A histiocitose de células de Langerhans (LCH) é uma doença rara, que se deve à acumulação anormal de células de Langerhans. Nas crianças, a LCH está associada a mau prognóstico, visto que tem, quase sempre, carácter multissistémico. Quando o pulmão é afectado, acumulam-se células de Langerhans em torno das pequenas vias aéreas, levando a dificuldade respiratória. Apresenta-se o caso de uma criança de 6 anos, do sexo feminino, que apresentou pneumotórax espontâneo. A tomografia torácica computorizada demonstrou múltiplas lesões quísticas bilaterais, com agravamento gradual e destruição da maior parte do parênquima. A biópsia pulmonar com marcação positiva para CD1a confirmou o diagnóstico de LCH. Dada a extensa invasão do parênquima pulmonar e a má resposta à terapêutica inicial, foi necessário administrar quimioterapia de segunda linha, no entanto, não foi conseguida melhoria significativa. Este caso ilustra a necessidade de um diagnóstico precoce e do desenvolvimento de terapêuticas mais eficazes.

Langerhans cell histiocytosis (LCH) is a rare disease, characterized by Langerhans cells accumulation. In children, LCH is associated with a bad prognosis, since it has almost always multissystemic character. When affecting the lung, Langerhans cells are gathered around small airways, leading to trouble breathing. We present a case of a female 6 years old child, who presented with spontaneous pneumothorax. Thoracic computed tomography scan showed multiple bilateral cystic lesions, with gradual worsening and destruction of most of the parenchyma. Lung biopsy staining positive for CD1a confirmed LCH diagnosis. Given the extent of lung parenchyma invasion and the lack of response to initial therapeutics, it was necessary to give second line chemotherapy. However it was not possible to achieve significant improvement. This case illustrates the need for an early diagnosis and the development of more effective therapies.
