Genes associados aos ritmos circadianos e cronoterapia

Abstract

Os ritmos circadianos influenciam muitos dos processos fisiológicos no corpo humano. Estão descritas variações circadianas no normal funcionamento dos sistemas do organismo humano, bem como na fisiopatologia de diversas doenças. A conjugação destes factores resultou na criação do conceito de Cronoterapia, que integra as características circadianas da farmacocinética e farmacodinâmica dos fármacos, mudando o paradigma da farmacologia. Uma terapêutica passa a considerar, não só a dose e a frequência de aplicação dos fármacos, mas também a altura do dia e a duração da administração. Muitas abordagens farmacológicas têm já em consideração as particularidades circadianas do corpo humano, gerando melhores resultados do que os regimes terapêuticos tradicionais equivalentes. Uma vez que os ritmos circadianos estão também intimamente relacionados com a replicação celular, existe um grande potencial numa evolução cronoterapêutica na área da quimioterapia. A presente monografia pretende então abordar os benefícios da aplicação da cronoterapia na eficácia e tolerabilidade dos agentes anticancerígenos, tendo o Irinotecano como exemplo primordial. As conclusões apresentadas indicam a terapia cada vez mais individualizada como o caminho a seguir, num campo terapêutico que deve ter em grande consideração a variabilidade genética inter-individual.
