Abstract

Introdução: O sucesso do tratamento endodôntico depende, entre outros fatores, da total eliminação dos microrganismos e a fase final deste tratamento será a obturação adequada do sistema canalar. O material mais utilizado para a obturação é a gutta-percha não podendo esta ser esterilizada por métodos térmicos por correr o risco de alterar as suas propriedades. Tanto o canal radicular como a superfície dos cones de gutta-percha (GP) contem bactérias que podem influenciar o sucesso do tratamento endodôntico. Objetivos: Os objetivos desta revisão de literatura são responder às questões se há interesse clinico em desinfetar os cones de GP antes da obturação e como o efetuar. Materiais e Métodos: No âmbito desta revisão da literatura, foi efetuada uma pesquisa de artigos científicos com recurso às bases de dados Pubmed e ScienceDirect. Resultados: Os cones de GP, quando mal manipulados, apresentam contaminação bacteriana no momento da obturação. A desinfeção química dos cones de GP pode ser feita com NaOCl, CHX, glutaraldeído e ácido paracético. O NaOCl e a CHX eliminam bactérias resistentes da superfície dos cones, variando com as concentrações e o tempo de desinfeção. O glutaraldeído necessita de mais tempo para eliminar as espécies bacterianas e o ácido paracético apresenta bons resultados em curtos períodos de tempo e contra vários tipos de patogénos. Conclusão: A desinfeção dos cones de GP tem interesse clinico pois elimina os microrganismos da superfície do cone que podem contaminar o sistema de canais. Trata-se de uma etapa adicional mas favorável ao tratamento endodôntico. O NaOCl e a CHX são eficazes na eliminação de bactérias na superfície dos cones a 1 minuto e 30 segundos, respetivamente. O ácido paracético apresenta menos efeitos indesejáveis e é eficaz contra diferentes espécies em apenas 1 minuto de atuação. São necessárias mais pesquisas sobre o tempo e concentração do desinfetante, tal como sobre os efeitos indesejáveis do desinfetante na superfície dos cones de GP e nas suas propriedades físicas.

Introduction: The success of the endodontic treatment depends of the total elimination of the microorganisms and the final purpose will be the appropriate filling of the canalar system. The most utilized material for the filling is the gutta-percha and it can’t be sterilized with thermal methods because it can change its properties. So the radicular canal as the points surface of the GP contain bacteria that could influence the success of the endodontic treatment. Purposes: The purposes of this review are to answer the questions if there is clinic interest to disinfect the GP points before the filling and how to do it. Materials and Methods: In the scope of this literature review, was made a research of scientific articles with resource of ScienceDirect and Pubmed database. Results: The GP points, when incorrectly manipulated, present bacterial contamination at the moment of the obturation. The chemical disinfection of the GP points can be done with NaOCl, CHX, glutaraldehyde and peracetic acid. The NaOCl and the CHX eliminate resistant bacteria of the points surface, ranging the concentration and the disinfection time. The glutaraldehyde needs more time to eliminate the same bacteria and the peracetic acid introduces good results in short periods of time and against different types of pathogens. Conclusion: The disinfection of the GP points has clinic interest because it eliminates the microorganisms of the surface of the points that may contain the canals system. It is an additional stage more favorable to the endodontic treatment. The NaOCl and the CHX are efficient in the elimination of the bacteria on the points surface to 1 minute and 30 seconds, respectively. The peracetic acid has less undesirable effects and it is efficient against different species in only 1 minute of operation. It is necessary more researches about the time and the disinfectant concentration, such as the undesirable effects of the disinfectant in the surface of the GP points and its physical properties.
