Abstract

O conhecimento aprofundado sobre as técnicas anestésicas existentes é essencial na prática clínica, pois o controlo da dor é um dos aspetos fundamentais nos cuidados prestados aos pacientes. De entre as técnicas anestésicas existentes, esta revisão da literatura irá destacar a anestesia intraóssea (IO). A anestesia IO envolve a injeção do anestésico local diretamente no osso esponjoso adjacente ao dente a ser anestesiado. Esta técnica tem aplicação clínica quer numa abordagem primária, quer como anestesia complementar. Os objetivos desta revisão bibliográfica são: conhecer as vantagens e desvantagens da técnica de anestesia IO; a sua eficácia enquanto medida primária e complementar na anestesia local em medicina dentária, assim como as suas possíveis complicações. Foi realizada uma pesquisa da literatura nas bases de dados PubMed e ScienceDirect e em livros na biblioteca da Faculdade de Medicina Dentária da Universidade de Lisboa. Dessa pesquisa selecionaram-se 47 artigos científicos relacionados com o tema. Pode-se concluir que a injeção IO permite uma anestesia eficaz alternativa às técnicas convencionais e desempenha um papel importante na anestesia complementar em pacientes com pulpite irreversível. O efeito secundário mais frequente é o aumento da frequência cardíaca com a injeção intraóssea de anestésico contendo vasoconstritor.

A deep knowledge about the existing anesthetic techniques is essential in clinical practice given that pain control is one of the fundamental aspects of patient care. Amongst the existing anesthetic techniques, this literature review highlights the intraosseous anesthesia (IO). The IO anesthesia allows placement of a local anesthetic solution directly into the cancellous bone adjacent to the tooth to be anesthetized. Such technique has clinical application both as a primary approach, and as a supplemental anesthesia. The purpose of this literature review is to make known the advantages and disadvantages of IO anesthesia technique, its effectiveness as a primary and supplementary local anesthesia in dentistry, as well as possible complications related to its use. A literature research was performed in the Pubmed and ScienceDirect databases, as well as through books available in the library of Faculdade de Medicina Dentária da Universidade de Lisboa. As a result, 47 scientific papers related to this theme were selected. IO anesthesia can be considered an effective alternative to the classic techniques and it plays an important role in supplemental anesthesia on patients diagnosed with irreversible pulpitis. The most common side effect is an increase in heart rate with IO injection of vasopressor-containing anesthetic solution.
