Abstract

As manifestações extra-intestinais da Doença Inflamatória Intestinal (DII) afectam cerca de 25-35% das crianças e adultos e podem atingir vários órgãos ou sistemas. As manifestações renais estão descritas e são bem conhecidas, sendo as mais frequentes a nefrolitíase por cálculos de oxalato de cálcio, a uropatia obstrutiva e as fístulas entero-vesicais. A nefrite túbulo intersticial (NTI) que surge em doentes com Doença de Crohn (DC) é frequentemente atribuída ao tratamento com aminosalicilatos, agentes que têm nefrotoxicidade conhecida. No entanto, surgem cada vez mais casos descritos na literatura de NTI em doentes não expostos ao tratamento com salicilatos ou que apresentam granulomas na biópsia renal, a maioria dos quais associada a uma glomerulopatia esclerosante. Estes achados não são sugestivos apenas da nefrotoxicidade classicamente associada aos salicilatos e apontam para a possibilidade da NTI glomerular e granulomatosa se tratar de uma manifestação extra-intestinal da DC. O presente caso clínico reporta a história de um rapaz, actualmente com 17 anos, com diagnóstico de DC e com NTI granulomatosa como manifestação extra-intestinal da doença. Este caso clínico pretende mostrar a importância do reconhecimento desta manifestação extra-intestinal que, apesar de não ser frequente, pode ter complicações e implicações graves, nomeadamente a doença renal terminal com necessidade de hemodiálise ou mesmo transplante renal.

The extra-intestinal manifestations of inflammatory bowel disease (IBD) occur in up to 25-35% of children and adults and can affect many organs and systems. The renal manifestations are described and are well known, the most frequent being nephrolithiasis due to calcium oxalate stones, obstructive uropathy and enterovesical fistula. The tubulointerstitial nephritis (TIN) that arises in patients with Crohn's disease (CD) is often attributed to treatment with aminosalicylates, which are known for their nephrotoxicity. However, there is an increasingly number of cases in the literature that describe TIN occuring in patients who were not exposed to treatment with salicylates or who have granulomas on renal biopsy, most of them also associated with a sclerosing glomerulonephritis. These findings are not suggestive of the classical nephrotoxicity associated with salicylates and point to the possibility of glomerular and granulomatous TIN being an extra-intestinal manifestation of CD. This case reports the story of a boy, now aged 17, diagnosed with CD who has glomerular and granulomatous TIN as an extra-intestinal manifestation of the disease. This case is intended to show the importance of recognizing this extraintestinal manifestation that, although not being frequent, can have serious complications and implications including end-stage renal disease requiring hemodialysis or even renal transplantation.
