Efeito da quimioterapia adjuvante na incidência de diarreia após encerramento de ileostomia de protecção em doentes com ressecção anterior do recto

Abstract

Objectivo: Determinar o efeito do tratamento adjuvante (capecitabina versus 5-FU) na incidência de diarreia grau 3/4 nos 30 dias após encerramento de ileostomia de protecção da anastomose colo-rectal (em doentes submetidos a ressecção anterior do recto). Métodos: Foram revistos retrospectivamente os doentes com tumor do recto em qualquer estadio, submetidos a ressecção anterior com criação de ileostomia de protecção, desde Janeiro de 2011 a Agosto de 2014, submetidos a terapêutica adjuvante com capecitabina ou 5-FU, independentemente da data de encerramento da ileostomia. Resultados: Foram incluídos 39 doentes (idade média de 62,8 anos, 71,8% do sexo masculino). 2 doentes receberam tratamento adjuvante com 5-FU e os restantes 37 doentes com capecitabina. 15 doentes apresentaram diarreia nos primeiros 30 dias após encerramento da ileostomia; desses, 60% apresentaram diarreia de grau 3. A duração média do tratamento adjuvante neste grupo de doentes foi de 15,2 semanas (versus 20,3 no grupo sem diarreia). O tempo de permanência da ileostomia foi semelhante em ambos os grupos, com ~87% mantendo a ileostomia por mais de 24 semanas. Conclusão: Não foi possível avaliar o efeito da quimioterapia adjuvante na incidência de diarreia após encerramento de ileostomia de protecção. Contudo, um menor tempo de duração do tratamento adjuvante com capecitabina parece estar associado a uma maior incidência de diarreia nos 30 dias após encerramento de ileostomia.

Objectives: Determine the effect of adjuvant regimen (capecitabine versus 5-FU) in the incidence of grade 3/4 diarrhea on the first 30 days after protection ileostomy closure after lower anterior resection for rectal cancer. Methods: Patients with rectal cancer (any stage) removed by lower anterior resection with creation of a protection ileostomy from January 2011 to August 2014 and that received adjuvant chemotherapy with capecitabine or 5-FU, regardless the date of ileostomy closure, were retrospectively reviewed. Results: 39 patients were included (mean age 62,8 years old; 71,8% male). 2 patients were treated in the adjuvant setting with 5-FU, while the other 37 were treated with capecitabine. 15 patients had diarrhea on the first 30 days after protection ileostomy closure, 60% of which had grade 3 diarrhea. Mean adjuvant treatment length in patients that presented with diarrhea was 15,2 weeks (versus 20,3 weeks for patients not presenting diarrhea after ileostomy closure). Ileostomy permanence time was similar in both groups, with ~87% keeping the ileostomy functioning over 24 weeks. Conclusions: It was not possible to evaluate the effect of adjuvant in the incidence of diarrhea after protection ileostomy closure. However, shorter adjuvant treatment course with capecitabine appears to be associates with higher incidence of diarrhea on the first 30 days after protection ileostomy closure.
