Leiomiomas uterinos e fertilidade

Abstract

Os leiomiomas uterinos são os tumores benignos mais frequentes em mulheres de idade fértil. Geralmente são assintomáticos, mas podem provocar menorragias, dor e pressão sobre estruturas pélvicas. Dependendo da sua localização, são associados a infertilidade: quanto maior o impacto no endométrio, maior o efeito negativo nas taxas de fertilidade. O diagnóstico e a caracterização de fibromiomas uterinos pode exigir a realização de ecografia pélvica, histerosonografia, histerossalpingografia, ressonância magnética ou histeroscopia diagnóstica. Atualmente, existem várias opções para o tratamento dos fibromiomas: hormonais, radiológicas e cirúrgicas, sendo a miomectomia considerada o “gold standard” para o tratamento de fibromiomas em pacientes inférteis ou que desejem engravidar. Pretende-se com este trabalho, rever os estudos que analisem impacto dos fibromiomas na fertilidade, abordem os métodos de diagnóstico e tratamentos disponíveis atualmente tendo sempre em conta a preservação de fertilidade.

The uterine leiomyomas are the most frequent benign tumors in childbearing age women. They are usually asymptomatic but may cause abnormal bleeding, pain and pelvic pressure. Depending on their location, they have been associated with infertility: the larger the impact on the endometrium, the greater the negative effect on fertility rates. The diagnosis and characterization of uterine fibroids may require a pelvic ultrasound, sonohysterography, hysterosalpingography, MRI or a diagnostic hysteroscopy. Currently, there are several options for the treatment of fibroids: hormonal, radiological and surgical, being the myomectomy the gold standard for fibroids treatment in infertile patients or desiring pregnancy. The aim of this review is to examine the studies which analyze the impact of fibroids on fertility and the currently available diagnosis and treatment methods with fertility preservation.
