Multimorbilidade : um caso clínico

Abstract

It is very common to deal with patients that present multiple pathologies simultaneously in the general practice consultation. Family doctors’ biggest challenge is to manage several diseases and their treatment, on each occasion having taken into consideration the social and family aspects of each patient as an individual. This report describes the case of an 87 year old woman, F.S., living alone, who suffers from more than one condition. A genogram is presented, alongside with the Lifeline of Medalie, with her most relevant life events. The list and problem map reveal multi-pathology. The clinical case description is based on two meetings with the patient and the access to her file, which can be found in the patient’s Health Center. In this report, a consideration about the medical therapy and a review of that therapy are provided. All the clinical reasoning is based on decision-making instruments. The aim of the report is to demonstrate the importance of the Clinical Method Focused on the Patient, as well as the Family Doctor’s as the care coordinator.

Lidar com doentes que apresentam várias patologias em simultâneo é muito comum na consulta de Medicina Geral e Familiar. O grande desafio dos Médicos de Família é saber gerir as várias doenças, os respetivos tratamentos, tendo sempre em consideração os aspetos familiares e sociais de cada doente, enquanto indivíduo. Descreve-se o caso de uma mulher de 87 anos, F.S., que vive sozinha, com várias patologias. É apresentado o seu genograma e a linha de vida de Medalie, com relevantes acontecimentos da sua vida. A lista e mapa de problemas revelam a sua multipatologia. A descrição deste caso clínico baseia-se em dois encontros com a doente e ainda no acesso ao seu processo, no Centro de Saúde onde a doente é seguida. É feita uma reflexão da terapêutica, bem como a revisão dessa mesma terapêutica. Todo o raciocínio clínico é baseado em instrumentos de decisão. Pretende-se demonstrar a importância do Método Clínico Centrado no Paciente e do Médico de Família, enquanto coordenador de cuidados.
