Abstract

O refluxo vesico-ureteral é consequente de um anormal desenvolvimento ou de um défice funcional a nível da junção uretero-vesical, associando-se a infecções do trato urinário (ITU) recorrentes, que, por sua vez, podem causar cicatrizes renais, hipertensão arterial crónica, doença renal crónica, entre outras morbilidades. Esta doença é a patologia urológica mais frequente na população pediátrica, afectando 30% das crianças com antecedentes de ITU. Os estudos feitos ao longo dos anos traduzem uma controvérsia e uma disparidade na abordagem a esta patologia que actualmente ainda permanece. Um dos tratamentos propostos é a profilaxia antibiótica que, considerando a elevada taxa de resolução espontânea do refluxo vesico-ureteral, tem como objectivo diminuir o risco de ITU recorrentes e evitar todas as consequências associadas a estas. Apesar de muitos ensaios e revisões sistemáticas defenderem a profilaxia antibiótica, é necessário ter em consideração as desvantagens desta terapêutica, nomeadamente as resistências bacterianas que poderão surgir. Assim, são analisados, revistos e discutidos os principais aspectos da profilaxia antibiótica, bem como a sua pertinência como abordagem a ser aplicada no refluxo vesico-ureteral, não esquecendo as limitações dos vários estudos e investigações.

Vesicoureteral reflux is a consequence of an abnormal development or a dysfunction in the ureterovesical junction, associated to recurrent urinary tract infections that, in turn, can cause renal scarring, hypertension chronic, chronic kidney disease and other morbidities. This disease is the most common urological disease in the pediatric population, affecting 30% of children with UTI antecedents. Several studies done over the years reflect a controversy and a disparity in the approach to this disease, which currently remains. One of the treatments proposed is antibiotic prophylaxis, that considering the high rate of spontaneous resolution of vesicoureteral reflux, aims to reduce the risk of recurrent UTI and avoid all the consequences associated. Although many trials and systematic reviews support antibiotic prophylaxis, it is essential to take into consideration the disadvantages of this therapy, specifically bacterial resistance that may arise. So are analyzed, reviewed and discussed the main aspects of antibiotic prophylaxis, as well as its relevance as an approach in vesicoureteral reflux, not disregarding the limitations of the various studies and trials.
