A distócia dinâmica e o lactato

Abstract

A progressão anormalmente lenta ou ausente do trabalho de parto, designada distócia, é um problema obstétrico comum sendo a indicação mais frequente para a realização de cesariana durante o trabalho de parto. O elevado recurso a este procedimento, implicando consequências importantes a nível de saúde pública e económico, pode resultar da ausência de consenso universal no que toca à definição e nomenclatura da distócia, critérios de diagnóstico e abordagem. Recentemente o lactato foi associado à distócia dinâmica, abrindo a possibilidade de colmatar algumas das carências referidas. É com base nesta hipótese que esta revisão foi efetuada tendo por objetivo rever os resultados de investigação em torno da distócia dinâmica e a sua relação com a presença de lactato no miométrio e no líquido amniótico. Verificou-se que o lactato no miométrio diminui a força das contrações miometriais sugerindo a sua provável contribuição para a distócia dinâmica. A concentração de lactato no líquido amniótico, reflexo do lactato no miométrio, pode ser facilmente medida e pode prever e possivelmente diagnosticar a distócia dinâmica. Foi recentemente apresentado, com resultados favoráveis, uma nova possibilidade terapêutica para a distócia dinâmica: o bicarbonato de sódio, dada a ocorrência em maior grau de acidose láctica miometrial nesta condição.

Slow or arrest of progress during labor, called dystocia, is a common obstetric problem being the main indication for intrapartum cesarean. The high use of this procedure, implying important consequences in terms of public health and economic, can be a result of the lack of universal consensus regarding the definition and nomenclature of dystocia, diagnostic criteria and approach. Recently lactate was associated with dynamic dystocia, opening the opportunity to overcome some of these shortcomings. It is based on this assumption that this review was carried out with the aim of obtaining information on the research results about the dynamic dystocia and its relationship with the presence of lactate in the myometrium and in amniotic fluid. It was found that lactate on the myometrium decreases the strength of myometrial contractions suggesting its possible contribution to dynamic dystocia. The concentration of lactate in amniotic fluid, as the reflection of the lactate on the myometrium, can be easily measured and can predict and possibly diagnose dynamic dystocia. It has recently been shown, with favorable results, a new therapeutic possibility for dynamic dystocia: the sodium bicarbonate, given the occurrence of myometrial lactic acidosis on a greater degree in this condition.
