Colangiocarcinoma intrahepático de células claras

Abstract

Clear cell intrahepatic cholangiocarcinoma is a rare variant of intrahepatic cholangiocarcinoma with only ten reported cases in the literature. This tumor needs to be differentiated from clear cell variant of hepatocellular carcinoma and other metastatic clear cell carcinomas. Hence, it is important to correctly diagnose this entity and differentiate it from other histologic mimics. We describe clinical, histopathological, and immunohistochemical findings of one such rare case in a 51-year-old woman with diabetes type II and systemic hipertension who was detected to have an intrahepatic tumor at routine ultrasound examination. After extensive workup for other primary neoplasms the patient underwent partial hepatectomy. On immunohistochemical staining, cytokeratin (CK) 7, CK8, CK18 and CK19 were positive, whereas CK20 was negative. The patient is currently free from disease at 12-months follow-up. A review of all the similar cases reported so far in the literature is also provided.

O Carcinoma intrahepático de células claras constitui uma variante rara de colangiocarcinoma intrahepático. Este tumor necessita ser diferenciado do carcinoma hepatocelular de células claras. Desta forma, é importante diagnosticar correctamente esta entidade e diferenciar de outros histologicamente semelhantes. Nesta revisão descrevem-se os achados clínicos, histopatológicos e imunohistoquímicos de um caso raro numa mulher de 51 anos com antecedentes de diabetes tipo II e hipertensão arterial, à qual foi detectada um tumor intrahepático em ecografia de rotina. Depois de extenso estudo para exclusão de outras patologias primárias, a doente foi submetida a hepatectomia. O estudo imunohistoquímico, foi positivo para citoqueratina (CK) 7, CK 8, CK 18 e CK 19, sendo negativo para CK20. A doente encontra-se actualmente livre de doença após 12 meses. Neste trabalho também se fará uma revisão de todos os casos clínicos reportados na literatura.
