Abstract

Depressive syndromes represent a particularly important dimension in the elder population. Due to the progressive aging of the Portuguese population, which accumulates comorbidities and is polymedicated, arises the need to provide for a specific approach in an attempt to increase the diagnostic rate of depression. It adds to an increase in morbidity and mortality with social, cognitive and physical decline, contributing to a high suicidal rate. Attending to the high prevalence and substancial costs it becomes essential to wage on the prevention and tailored interventions, improving patient access to medical services. Advances in psychiatry, neurobiology and imaging allow us a thorough knowledge of pathophysiological changes in depressed mood disorders. The therapeutic arsenal is extensive and effective. With an early diagnosis, appropriate treatment combined with psychological and behavioral interventions medicine has much to offer to these patients. Occupying a prominent place among the most debilitating conditions, a concerted approach in providing and building a better trained social workers, authorities and health professionals is desirable. This literature review update approaches the most relevant aspects of "Depression in the Elderly" with a special focus at the national level and in the context of primary care physicians.

As síndromas depressivas assumem na população idosa uma dimensão preponderante. Assistindo-se ao progressivo envelhecimento da população portuguesa que acumula comorbilidades e é polimedicada, urge atentar nas especificidades desta na tentativa de incrementar a taxa diagnóstica da depressão. Ela condiciona um aumento da morbilidade e mortalidade com declínio social, cognitivo e físico, contribuindo para uma alta taxa suicidária. Pela elevada prevalência, custos avultados torna-se essencial a prevenção e adequar as intervenções e facilitar o acesso aos serviços. Os avanços na psiquiatria, neurobiologia e imagiologia permitem-nos hoje um aprofundado conhecimento fisiopatológico das perturbações de humor depressivo. O arsenal terapêutico é extenso e eficaz. Com um diagnóstico precoce, tratamento adequado conjugado com intervenções psicológicas e comportamentais a medicina tem muito para oferecer a estes doentes. Ocupando um lugar de destaque entre as patologias mais debilitantes, uma abordagem concertada de capacitação dos profissionais de acção social, das autoridades e dos profissionais de saúde é desejável. Esta revisão bibliográfica trata de forma actualizada os aspectos mais relevantes da “Depressão no Idoso” com especial enfoque a nível nacional e na óptica do médico de cuidados primários.
