Abstract

Introdução: A trombose venosa profunda (TVP) neonatal é uma entidade rara e importante na criança, sendo nesta faixa etária mais frequente no recém-nascido, consequência de particularidades da fisiologia da hemostase neonatal e da presença de fatores de risco maternos, neonatais adquiridos ou genéticos. A localização mais frequente, os sinais e sintomas, o diagnóstico e a terapêutica da TVP serão abordados neste trabalho. Objetivo: Descrever um caso clínico de TVP secundária a sépsis a Streptococcus pyogenes. O caso apresentado procura salientar os aspetos práticos mais importantes do diagnóstico e do tratamento de TVP. Caso Clínico: Apresenta-se o caso de uma recém-nascida com 7 dias de vida, previamente saudável, internada por febre, dor, edema e alteração da coloração do membro inferior esquerdo. Foi-lhe diagnosticada sépsis com meningite a Streptococcus pyogenes complicada de TVP das veias popliteia esquerda, femorais profunda e superficial, ilíaca externa esquerda, ilíaca comum esquerda e veia cava inferior. Cumpriu 21 dias de antibioterapia, com resolução do quadro infecioso. Realizou terapêutica anticoagulante com enoxaparina, com resolução do quadro de obstrução venosa. Teve alta ao 21º dia de internamento. Planeou-se terapêutica anticoagulante por 6 meses e avaliação nas consultas de Desenvolvimento, Otorrinolaringologia e Oftalmologia. Aos 8 meses de idade apresentava-se com bom estado geral e exame objetivo normal. Conclusões: A terapêutica anticoagulante na TVP neonatal tem sido alterada nos últimos anos, baseada na experiência adquirida no tratamento do adulto. A enoxaparina é atualmente a terapêutica de eleição, não havendo experiência suficiente para a utilização de novos fármacos com segurança no recém-nascido. A necessidade de investigação na área pediátrica é uma realidade, principalmente em relação aos fatores de risco mais prevalentes em Portugal e à terapêutica desta patologia neste grupo etário.

Introduction: Deep vein thrombosis (DVT) is a rare and important neonatal entity in children, among this age group is most common in newborns, a consequence of the particular physiology of neonatal hemostasis and the presence of maternal, acquired neonatal or genetic risck factors. The most frequent location, signs and symptoms, diagnosis and treatment of DVT will be addresses in this work. Objective: To describe a case of DVT secondary to sepsis from Streptococcus pyogenes. The case presented seeks to highlight the most important practical aspects of diagnosis and treatment of DVT and the sepsis context in which that entity appears. Case Report: We report the case of a 7 days old newborn, previously healthy, hospitalized for fever, pain, swelling and discoloration of the left leg. Sepsis was diagnosed, with meningitis from Streptococcus pyogenes, and DVT of the left popliteal veins, deep and superficial femoral veins, left external iliac vein, left common iliac vein and inferior cava vein. She acomplished 21 days of antibiotic therapy, with resolution of the infectious situation. Held anticoagulant therapy with enoxaparin, with a resolution of venous obstruction. She was discharged after 21 days of hospitalization. Anticoagulant therapy was planned for 6 months and review in the Development, Otolaryngology and Ophthalmology outpacient clinics. At 8 months of age the patient was in good general state and the physical exam was normal. Conclusions: Anticoagulant therapy in neonatal DVT has been changed in the last years, based on the experience in adult treatment. Enoxaparin is currently the preferred therapy, as there is insufficient experience for the use of new drugs safely in the newborn. The need for research in the pediatric area is a reality, especially regarding the most prevalent risk factors in Portugal and treatment of this pathology in this age group.
