Abstract

Introdução: A anquiloglossia é uma das patologias que pode ser detetada à nascença, sendo muito importante que o médico dentista a avalie e diagnostique, informando os pais da necessidade do seu tratamento. Objetivo: Realizar uma revisão literária acerca dos critérios de diagnóstico e classificações da anquiloglossia, o impacto desta nos doentes pediátricos e avaliar a indicação de tratamento cirúrgico ou conservador, tendo em conta a sua eficácia, idade ideal de execução, riscos e complicações. Métodos: Para a realização deste trabalho de revisão bibliográfica procedeu-se a uma pesquisa na base de dados PUBMED/MEDLINE/SCIELO que incluiu artigos publicados em Espanhol, Inglês e Português entre o período de 2000–2014. Para além destes, foram também acrescentadas publicações a partir da leitura dos artigos previamente selecionados. Discussão: A definição de anquiloglossia ainda não é consensual na comunidade médica, tal como uma classificação que padronize o diagnóstico e as indicações de tratamento. A anquiloglossia causa problemas de amamentação no recém-nascido e na mãe que favorecem o desmame precoce quando a frenotomia não é realizada precocemente. Na criança, inicialmente surgem problemas de mastigação e posteriormente, problemas de fala, da motricidade lingual e sociais que melhoram com a frenectomia. É aconselhada a prévia avaliação pela Terapia da fala. Conclusão: Ao longo da sua história natural, desde o recém-nascido ao adolescente, a anquiloglossia provoca diversos distúrbios solucionáveis com procedimentos cirúrgicos como a frenotomia e frenectomia em complemento de outros tratamentos, como a terapia da fala.

Introduction: Ankyloglossia is a pathology that can be detected very early after birth. It is mandatory that the patient be presented to a dentist in order to make a diagnostic evaluation. The child’s parents should be then informed about the advantages and need of treatment at that time. Objectives: The author proceeded to an evaluation of the diagnostic criteria and classifications of ankyloglossia and its impact on pediatric patients. She evaluated the conservative and surgical treatment indications according to their efficacy, ideal age for treatment, risks and complications. Methods: The author proceeded to a bibliographic research on PUBMED/MEDLINE/SCIELO online database including articles in Portuguese, Spanish and English between 2000 and 2014. Beyond these, articles of previously read selected publications were added. Discussion: The ankyloglossia definition is not consensual in the medical community, nor is a classification defining the diagnostic and treatment indications. Ankyloglossia disturbs breastfeeding favoring its early interruption if a timely frenotomy is not performed. In childhood, chewing troubles arise soon. Speaking troubles, tongue mobility and social issues ensue after. All seem to improve with frenectomy. Previous speech therapy is advised. Conclusion: Ankyloglossia causes a number of problems along its natural history, from newborn to adolescence, which have can be solved with surgical procedures like frenotomy or frenectomy. Complementary treatments as speech therapy can and should be used along.
