Abstract

Descrita como a hipomineralização de origem sistémica de um a quatro primeiros molares permanentes envolvendo com frequência incisivos permanentes igualmente afetados, nos últimos anos, a Hipomineralização incisivo-molar tem vindo a ganhar uma maior notoriedade na prática clínica dos médicos dentistas. Esta pode ser identificada logo após o início da erupção dos molares ou incisivos permanentes. Quer pela sintomatologia e comportamento por vezes apresentados pelos jovens pacientes, quer pelas propriedades reduzidas do esmalte hipomineralizado a que frequentemente estão associados insucessos no tratamento restaurador, esta condição constitui um enorme desafio aquando da abordagem terapêutica. A hipersensibilidade aliada à dificuldade em anestesiar os dentes afetados condiciona muitas vezes a higiene oral das crianças e o tratamento adequado, resultando em problemas de medo, ansiedade e de controlo de comportamento, comprometendo a saúde oral destes pacientes. A ocorrência da fratura do esmalte pós-eruptiva e a localização atípica das lesões fazem com que a abordagem terapêutica clássica, para lesões de cárie, não seja a mais adequada nos pacientes afetados. Os tratamentos podem ir desde tratamentos preventivos até à extração do dente. Os poucos estudos existentes e a falta de índices de classificação e metodologias padronizados contribuem para a baixa evidência científica existente em relação à Hipomineralização incisivo-molar. Na literatura são sugeridas várias linhas de orientação para a escolha do melhor tratamento, sendo preconizado o diagnóstico precoce e o controlo regular destes pacientes para a promoção de uma boa saúde oral.

Described as the hypomineralization of sistemic origin of one to four first permanent molars, often associated with similarly affected permanent incisors, Molar incisor hypomineralization, in recent years, has gained greater attention in the dental clinical practice. It can be identified soon after the beginning of the eruption of the permanent molars or incisors. Whether by the symptoms and behavior that sometimes appear in the young patients, either by reduced properties of the hypomineralized enamel, which often is associated with failure of the restorative treatment, this condition is a major challenge in terms of treatment approach. Hypersensivity allied with difficulties in obtaining adequate anesthesia often affects the oral hygiene of children and the proper treatment, resulting in fear, anxiety and behavior problems, compromising the oral health of these patients. The enamel post-eruptive breakdown and the atypical location of lesions make the traditional treatment approach, for the carious lesions, inappropriate for these patients. Treatment can range from preventative treatments to tooth extraction. The few existing studies and the lack of classification indexes and standardized methodologies contribute to a low scientific evidence regarding hypomineralization incisor-molar. The literature suggests several guidelines when choosing the best treatment, the early diagnosis and the regular monitoring of these patients are essential to promote good oral health.
