Abstract

A miocardiopatia cirrótica (MCC) é uma complicação cardiovascular da cirrose, recentemente reconhecida. As suas principais características incluem função sistólica comprometida face ao stress, disfunção diastólica e alterações electrofisiológicas. É independente da etiologia e da gravidade da DHC. Clinicamente inaparente em repouso, torna-se evidente quando os doentes são sujeitos a condições de stress, nomeadamente procedimentos invasivos, como TIPS ou transplante hepático. Dada a clínica subtil, é frequentemente subdiagnosticada. Neste trabalho descreve-se o caso de um homem de 70 anos, com diagnóstico de cirrose hepática desde há seis anos, que no seguimento da colocação de TIPS por ascite refractária, apresenta um agravamento do estado inicial com sinais de sobrecarga de volume. A propósito deste caso pretende-se realizar uma revisão da literatura sobre esta entidade, cuja importância clínica é crescente e para a qual um alto índice de suspeição permanece a maior ferramenta diagnóstica.

Cirrhotic cardiomyopathy is a recently acknowledged cardiovascular complication of cirrhosis. The main features include impaired systolic function facing stress, diastolic dysfunction and electrophysiological changes. It was proven independent of the etiology and severity of CLD. Clinically unapparent at rest, it becomes overt when patients undergo stressful situations such as invasive procedures like TIPS or hepatic transplant. Considering the subtle clinic, it is frequently undiagnosed. This paper presents the case of a 70 years old male, with the diagnosis of cirrhosis for six years, who shows a deterioration of the initial status with signs of volume overload following TIPS placement for refractory ascites. Taking this case it is intended to perform a literary review of this entity, whose clinical importance is growing and for which a high level of suspicion remains the best diagnostic tool.
